摘要
目的:探讨不同术式对基底节区高血压脑出血患者的临床治疗,以期改善患者的预后。方法:选取2008年1月-2015年12月本院收治的基底节区高血压脑出血患者300例,共分为三组,其中穿刺引流术组(A组)104例,小骨窗开颅血肿清除术组(B组)101例,开颅血肿清除、去骨瓣减压术组(C组)95例,分析三组患者以不同手术方式治疗基底节区高血压脑出血的预后状况。结果:A组患者清醒和轻度意识障碍例数分别占总例数的79.81%和18.27%,高于B组的12.87%、54.46%和C组的0、5.26%,组间比较差异均有统计学意义(P<0.05);从血肿清除效果来看,A组最差,明显低于B、C两组,在术后并发症和死亡率方面三组逐渐递增,差异均有统计学意义(P<0.05)。结论:结合患者的年龄、病情轻重及颅内血肿量的多少选择不同的手术方式可以提高患者的预后,改善生活质量。
Objective:To analyze and improve the clinical treatment experience of patients with hypertensive cerebral hemorrhage in basal ganglia region.Method:300 cases of patients with hypertensive basal ganglia intracerebral hemorrhage were divided into three groups from January 2008 to December 2015 in our hospital,including 104 cases of the puncture drainage group(group A),101 cases of the removal of small bone window craniotomy hematoma group(group B) and 95 cases of the hematoma removal to bone flap craniotomy decompression group(group C),the 300 cases with surgical indications of basal ganglia hypertensive cerebral hemorrhage patients with prognosis of different surgical methods were analyzed.Result:The number of patients in group A were 18.27% and 79.81% of the total number of the patients with mild consciousness,significantly higher than that of group B 12.87%,54.46% and 0,5.26% of group C,the differences were statistically significant among the groups(P〈0.05).From the point of hematoma removal effect,the group A was lower than that in group B and C,the postoperative complications and mortality in three groups were increased gradually,the differences were statistically significant among the groups(P〈0.05).Conclusion:According to the patient's age, the severity of the disease and the amount of combined intracranial hematoma to choose a different surgical approach can improve the prognosis of patients and the quality of life.
