跟骨关节内骨折切开复位内固定是否植骨的临床疗效对比研究 被引量:6

摘要 目的:探讨跟骨关节内骨折切开复位内固定手术中植骨与不植骨的临床疗效。方法:选取本院2012年1月-2014年10月的跟骨关节内骨折行切开复位内固定手术的患者共52例,按照随机数表法分为对照组与观察组,对照组27例采用切开复位内固定加植骨手术治疗,观察组25例采用不植骨切开复位内固定手术治疗。比较两组患者手术持续时间,住院治疗费用,骨折愈合时间,术后1、3、6个月的Maryland评分、Bohler角度及跟骨高度。结果:观察组手术持续时间及住院治疗费用明显低于对照组,比较差异有统计学意义(P<0.05)。观察组患者骨折愈合时间为(13.32±2.54)周,对照组为(13.56±3.42)周,比较差异无统计学意义(P>0.05)。两组患者术后1、3、6个月Maryland评分及Bohler角度比较,差异无统计学意义(P>0.05)。观察组术后1、3、6个月跟骨高度分别是(44.73±0.74)、(45.86±0.84)、(45.74±0.85)mm,与对照组同期跟骨高度比较,差异无统计学意义(P>0.05)。结论:在跟骨骨折切开复位内固定手术中无需植骨,该方法不但安全有效、临床效果良好,而且手术时间相对减少、明显降低医疗费用,值得临床推广。 Objective: To investigate the clinical efficacy of the calcaneus bone and no bone of intra-articular fractures open reduction and internal fixation surgery. Method: 52 patients from January 2012 to October 2014 with calcaneal fractures open reduction and internal fixation in our hospital were selected and divided into control group and observation group. The control group received open reduction and internal fixation used bone graft, and the observation group without bone graft surgery open reduction and internal fixation. Duration of surgery, hospitalization costs, fracture healing time, Maryland score,Bohler angle and calcaneal height after one month, three months, and six months between the two groups were compared. Result: Operative duration and cost of hospitalization of the observation group was significantly lower than those of the control group, the differences were statistically significant(P〈0.05). The fracture healing time of the observation group was(13.32±2.54) weeks, that of the control group was(13.56±3.42) weeks, there was no significant difference between the two groups(P〈0.05). After one month, three months, and six months, there were no significant differences in Bohler angle and Maryland score between the two groups(P〈0.05). After one month, three months, and six months, calcaneal height of the observation group was(44.73±0.74) mm,(45.86±0.84) mm,(45.74±0.85) mm respectively, and there was no significant difference compared with the control group over the same period(P〈0.05). Conclusion: Open reduction and internal fixation of calcaneal fractures without bone graft surgery is not only safe and effective, has good clinical effect, but also reduces operative time and decreases medical costs, which is worthy of promotion.
