脂蛋白相关磷脂酶A_2测定对冠心病风险判断的价值 被引量:3

The value of lipoprotein associated phospholipase A_2 assay for the assessment of coronary heart disease risk

摘要 目的探讨血清脂蛋白相关磷脂酶A2(Lp-PLA_2)水平和冠心病患者冠状动脉狭窄程度的关系及其在判断冠心病预后时的价值。方法根据冠状动脉造影检查结果,将843例冠心病和疑似冠心病患者分为高危风险组(至少1支冠状动脉狭窄程度>50%)和低危风险组(单支冠状动脉狭窄程度≤50%),采用ELISA方法检测受试者Lp-PLA_2水平,对结果进行对比分析。结果冠心病高危风险组的吸烟患者比例、SBP、TG、LDL-C、糖尿病患病率高于低危风险组,HDL-C显著低于低危风险组,差异均有统计学意义(P<0.05);高危风险组受试者血清Lp-PLA_2水平显著高于低危风险组,差异有统计学意义(P<0.01);Lp-PLA_2判断冠心病高危患者的灵敏度为94.5%,特异度为91.4%,准确度为92.5%。结论血清Lp-PLA_2检测可反映冠状动脉狭窄严重程度,并对冠心病风险判断有一定的预测价值。 Objective To discuss the relationship between the serum level of lipoprotein- associated phospholipase A2( Lp-PLA2) and the arteriostenosis degree of coronary heart disease( CHD),as well as its value of judging the prognosis of coronary heart disease. Methods 843 patients were divided into high risk group( degree of coronary artery stenosis 50% in at least one artery) and low risk group( single degree of coronary artery stenosis ≤50%). The serum levels of Lp- PLA2 were detected by ELISA method,and the results were analyzed and compared. Results The proportion of smoking patients,SBP,TG,LDL- C,the prevalence of diabetes mellitus in the high risk subjects were significantly higher than these in the low- risk group,HDL- C in the high risk subjects was significantly lower than that in the low- risk group,and the differences were statistically significant( P〈 0. 05); the serum level of Lp- PLA2 in high risk subjects were significantly higher than that in the low- risk group,the difference was statistically significant( P 〈0. 01); Sensitivity of Lp- PLA2 in CHD patients was 94. 5%,the specificity was 91. 4%,and the accuracy was 92. 5%. Conclusion The serum level of Lp- PLA2 can reflect the severity of coronary artery disease,and it is of good value to predict the risk of coronary heart disease patients.
