摘要
目的探讨骨髓干细胞移植联合伞状支撑骨移植术治疗股骨头坏死(osteonecrosis of the femoral head,ONFH)的临床效果。方法 ONFH患者48例(58髋),其中30例(36髋)采用骨髓干细胞和伞状支撑骨移植术为观察组,18例(22髋)行单纯股方肌骨柱移植术为对照组;比较2组术后百分法评分和影像学检查结果。结果观察组与对照组术前百分法评分[(56.06±6.88)、(55.54±6.36)分]比较差异无统计学意义(P>0.05);术后随访18个月,2组百分法评分均较术前提高,且观察组评分[(87.47±5.66)分]高于对照组[(76.14±7.57)分],治疗优良率(91.67%)高于对照组(72.7%)(P<0.05);观察组术后植骨愈合时间[(3.9±0.5)个月]和MRI坏死面积百分比[(9.19±5.84)%]低于对照组[(5.2±0.6)个月、(14.05±5.96)%],差异均有统计学意义(P<0.05)。结论骨髓干细胞和伞状支撑骨移植术治疗ONFH可加速成骨,更好恢复股骨头的生物力学、防止股骨头塌陷,近期效果良好。
Objective To explore the therapeutic effect of transplantation of bone marrow stem cells and umbrella strut bone grafting on osteonecrosis of the femoral head (ONFH). Methods Forty-eight adult patients (58 hips) with ONFH were divided into observation group receiving transplantation of bone marrow stem cells and umbrella strut bone grafting (30 patients, 36 hips) and control group receiving simple unit side muscle bone flap grafting (18 patients, 22 hips). The percentage score and radiologic results were compared between two groups. Results There was no significant difference of the percentage score between observation group (56.06 ± 6.88) and control group (55.54± 6.36) before treatment (P〈0.05). After follow-up for 18 months, the percentage scores were found higher in both two groups, and it was significantly higher in observation group (87.47±5.66) than that in control group (76.14±7.57). The rate of excellent and good results was higher (91.67%), average healing time was shorter ((3.9±0.5) months) and the percentage of the coagulation region by MRI was lower ((9. 19 ± 5. 84)%) in observation group than those in control group (72. 7% (5.2±0. 6) months, (14. 05 ± 5. 96)%) (P〈0.05). Conclusion Transplantation of bone marrow stem cells and umbrella strut bone grafting could effectively accelerate osteogenesis, restore the normal biomeehanics property of femoral head and prevent collapse of the femoral head for ONFH, with satisfactory short-term effect.
