动态肌电图指导下电刺激治疗脑卒中后偏瘫的临床价值

摘要 目的:探讨动态肌电图指导下电刺激治疗脑卒中后偏瘫的临床价值。方法:2014年6月—2015年8月神经内二科共收治80例脑梗塞后肢体偏瘫患者,按随机数字法分为两组,随机分为观察组(40例)和对照组(40例),对照组给予常规康复治疗,观察组采用动态肌电图指导下电刺激治疗,分析上肢周围神经的电学变化,并分析其与肌力变化的相关性。结果:治疗后尺神经、桡神经、正中神经的传导速度觉较治疗前明显加快,但观察组快于对照组,其组内和组间差异均有统计学意义(p<0.05);观察组肌力IV级以上百分比明显高于对照组(p<0.05)。结论:动态肌电图指导下电刺激治疗卒中后遗留肢体偏瘫患者,可有效促进周围神经再生及神经功能恢复,改善患肢肌力。 Objective: To investigate the clinical value of dynamic EMG guided by electric stimulation in the treatment of hemiplegia after stroke. Methods: 80 patients with cerebral infarction were randomly divided into observation group(40 cases) and control group(40 cases), and the observation group was treated with dynamic EMG guided by electric stimulation, and the correlation between the changes of-2015 and muscle strength was analyzed. Results: the conduction velocity of ulnar nerve, radial nerve and median nerve were significantly higher than those before treatment, but the observation group was faster than that of control group(p〈0.05), and the percentage of muscle strength in the observation group was significantly higher than that in control group(p〈0.05). Conclusion: dynamic EMG guided by electric stimulation in the treatment of stroke patients with hemiplegia after stroke can effectively promote peripheral nerve regeneration and nerve function recovery, improve the limb muscle strength.
