摘要
目的 :探讨成人急性髓系白血病M2型(acute myeloid leukemia-type M2,AML-M2)白血病细胞免疫表型的特征及临床意义。方法 :采用多参数FCM和CD45/侧向角散射(side scatter,SSC)设门,对93例AML-M2患者的白血病细胞进行免疫表型分析。结果 :93例AML-M2患者骨髓白血病细胞表达髓系祖细胞抗原CD117(91.4%)、CD34(80.6%)和HLA-DR(46.2%)及髓系相关抗原CD13(86.0%)、CD33(67.7%)和CD71(50.5%)。部分患者伴有淋系分化抗原的表达,CD7阳性表达率最高(31.2%),其次为CD56(25.8%)。17例RUNX 1-RUNX 1T 1融合基因阳性AML-M2患者白血病细胞阳性表达率最高的抗原为CD34(76.4%),其次为CD33(70.6%)、CD117(70.6%)和CD13(52.9%)。76例RUNX1-RUNX1T1融合基因阴性AML-M2患者白血病细胞高表达CD117(96.1%)、CD13(93.4%)、CD34(81.6%)、CD33(67.1%)和CD71(55.3%)。在RUNX 1-RUNX 1T 1融合基因阳性患者中,阳性表达率最高的淋系分化抗原为CD56(47.1%);在RUNX1-RUNX 1T 1融合基因阴性患者中,阳性表达率最高的淋系分化抗原为CD7(35.5%)。RUNX1-RUNX1T1融合基因阳性患者的CD117、CD13和CD71阳性表达率低于RUNX 1-RUNX 1T 1融合基因阴性患者(P值均<0.05),而CD38和CD56阳性表达率均高于RUNX1-RUNX1T1融合基因阴性患者(P值均<0.05)。结论 :AML-M2白血病细胞抗原表达复杂,异质性强,强表达髓系相关抗原并常伴有淋系分化抗原的表达。RUNX1-RUNX1T1融合基因阳性与阴性AML-M2患者表现出不同的免疫表型特征。
Objective:To investigate the immunophenotypic features of leukemia cells in patients with acute myeloid leukemia-type M2(AML-M2) and its clinical significance.Methods:CD45/side scatter(SSC) gating strategy of FCM was used to determine immunophenotyping of leukemia cells in 93 patients with AML-M2.Results:The expression rates of hematopoietic stem/progenitor cell differentiation antigens,such as CD 117(91.4%),CD 34(80.6%) and HLA-DR(46.2%),were high.The myeloidassociated antigens of CD13(86.0%),CD33(67.7%) and CD71(50.5%) were also highly expressed.In some patients,lymphoid-associated antigens were expressed,in which,the positive expression rate of CD7(31.2%) was highest,followed by CD56(25.8%).In17 RUNX1-RUNX1T1-positive patients,the positive expression rate of CD34(76.4%)was highest,followed by CD33(70.6%),CD117(70.6%) and CD13(52.9%).In 76 RUNX1-RUNX1T1-negative patients,the positive expression rates of CD117(96.1%),CD13(93.4%),CD34(81.6%),CD33(67.1%) and CD71(55.3%) were high.Of lymphoid-associated antigens,the positive expression rate of CD56(47.1%) was highest in RUNX1-RUNX1T1-positive patients,and the positive expression rate of CD7(35.5%) was highest in RUNX1-RUNX1T1-negative patients.The positive expression rates of CD117,CD13 and CD71 were lower and the positive expression rates of CD38 and CD56 were higher in RUNX1-RUNX1T1-positive patients than those in RUNX1-RUNX1T1-negative patients(all P 〈 0.05).Conclusion:The antigen expression in AML-M2 is complicated with high heterogeneity,mainly presented with high expression of myeloid-associated antigens and expression of lymphoidassociated antigens.The immunophenotypic features between RUNX1-RUNX1T1-positive and-negative AML-M2 patients are different.
