摘要
目的探讨和评价降钙素原(PCT)在细菌性感染诊断和治疗效果评价中的价值。方法选取细菌培养阳性的身体不同部位感染患者的血清,采用酶联荧光分析法(ELFA)检测这些细菌感染患者血清PCT水平,同时检测同期病毒感染者和健康者的血清PCT水平作为对照。结果健康者(30例)和病毒感染者(50例)PCT水平极低(0.0-0.10ng/m1);皮肤组织感染21例,PCT水平:0.32-0.58ng/ml,平均0.38ng/ml;上呼吸道细菌感染31例,PCT水平:0.4300.75ng/ml,平均0.52ng/ml;细菌性肺炎37例,PCT水平:0.69-1.63ng/ml,平均1.25ng/ml;细菌性泌尿生殖道感染43例,PCT水平:O.57-1.41ng/ml,平均0.94ng/ml;细菌性心内膜炎19例,PCT水平:1.23-1.98ng/ml,平均1.46ng/ml;阑尾炎患者27例,PCT水平:0.54-0.93ng/ml,平均0.71ng/ml;脓毒血症患者25例,PCT水平:2.32-10.24ng/ml,平均4.26ng/ml;25例脓毒血症患者中有6例,PCT水平持续〉6.0ng/ml,治疗2周后死亡。结论PCT测定对于细菌性感染是一个敏感的指标,可以作为细菌感染性疾病的诊断和鉴别诊断,同时也可作为临床疗效评价的一个指标。
Objective To investigate clinical value of calcitonin original (PCT) for diagnosis and treatment evaluation of bacterial infections. Methods Select germiculture positive infection in patients with serum at different positions of the body, using the method of enzyme-linked fluorescent (ELFA) to detect the bacteria infection patients serum PCT levels, while detecting serum PCT level of infected and healthy subjects as controls. Results Healthy subjects (30 cases) and very low virus infection (50 cases) PCT (0.0-0.10ng/ml); Skin tissue 21 cases ofinfection,PCT levels: 0.32-0.58 ng/ml, with an average of 0.38 ng/ml;Bacterial infection of upper respiratory tract in 31 cases,PCT levels: 0.43-0.75 ng/ml, with an average of 0.52 ng/ml;Bacterial pneumonia in 37 cases, PCT levels: 0.69-1.63 ng/ml, with an average of 1.25 ng/ml;Bacterial urinary tract infection in 43 cases, PCT levels: 0.57-1.41 ng/ml, with an average of 0.94ng/ml;Bacterial endocarditis in 19 cases,PCT levels:1.23-1.98 ng/ml, with an average of 1.46ng/ ml;Appendicitis in 27 cases,PCT levels:0.54-0.93 ng/ml, with an average of 0.71ng/ml;25 patients with sepSis,PCT levels:2.32-10.24 ng/ml, with an average of 4.26ng/ml;Six of 25 patients with sepsis,PCT levels continue〉6.0ng/ml, death after 2 weeks of treatment. Conclusion PCT measurement for bacterial infection is a sensitive index, which can be used as the diagnosis and differential diagnosis of bacterial infectious diseases, but also can be used as an indicator of clinical curative effect evaluation.
