摘要
目的:观察^(125)放射粒子植入治疗颌面部恶性肿瘤的临床效果及患者生存率。方法:纳入颌面部恶性肿瘤的患者43例,按治疗计划系统(TPS)植入^(125)粒子,粒子间距1 cm,活度0.7m Ci,术后随访复查。结果:粒子植入顺利,平均41颗/人,术后未见粒子移位。随访率93.02%,治疗有效率(PR+CR)90.70%,靶区未见瘤复发,因瘤死亡率9.30%。患者总生存率74.43%,0.5、1、2、3年及5年累积患者生存率分别为:93.0%、85.7%、79.3、69.8%及56.9%,平均生存36.06~50.04个月,中位生存期43.05月,最长无瘤生存60月,1级放射损伤率20.93%,2例患者原有面神经损伤术后恢复。结论:^(125)粒子植入治疗颌面部恶性肿瘤组织损伤小,能提高患者生存率并保证口咽功能。
Objective： To investigate the effects of125 I radioactive particle implantation in the treatment of maxillofacial malignancy.Methods： 43 patients with maxillofacial malignancy were treated with125 I radioactive particle implantation. The procedure was carried out according to the treatment planning system（ TPS）,with the particles spaced uniformly at 1 cm intervals and with the activity of 0. 7m Ci by 41 particles per case on average. All patients were followed up for 6- 60 months. Results： The whole treatment procedure was successful,and no particle displaced. The follow-up rate was 93. 02% and treatment effective rate（ CR ＋ PR） was 90. 70%. No recurrence was found in all target areas. The mortality due to tumor was 9. 30% and total survival rate was 74. 43%. The cumulative survival rate of the patients in 0. 5,1,2,3 and 5 years was 93. 0%,85. 7%,79. 3,69. 8% and 56. 9% respectively. Survival period on average was 36. 06- 50. 04 months,with the median of 43. 05 months. The longest tumor-free survival period was 60 months. Radiation injury rate was 20. 93% and only level 1 radiation injury was observed in all the cases. Facial nerve dysfunction was found in 2 cases and recovered after treatment. Conclusion： Treatment of maxillofacial malignancy by implantation of125 I particles is convenient and minimally invasive. The treatment can increase survival rate of the patients and guarantee the oropharynxes＇ function.
