摘要
目的探讨大承气汤对肝脏缺血再灌注的影响及其机制。方法将24只SD大鼠随机分为3组,每组8只。假手术组,缺血再灌注组(I/R组),大承气汤预处理组。后两组用手术造重度失血性休克再灌注模型,使血压维持在5.3 k Pa(40 mm Hg)持续90 min。术后取肝脏标本,石蜡切片进行HE染色观察肝组织病理变化;应用i NOS特异性抗体免疫组化定性及半定量检测肝细胞i NOS的表达与分布。结果与假手术组比较,I/R组大鼠i NOS表达程度显著增高(P<0.01),组织切片显示I/R组大鼠肝细胞水肿,肝细胞灶性坏死伴大量中性粒细胞聚集,肝窦狭窄甚至闭塞等变化;而大承气汤组与I/R组大鼠比较,i NOS表达程度明显降低(P<0.01),大承气汤组大鼠肝细胞病理学改善。结论大承气汤能明显减轻大鼠失血性休克再灌注肝组织的细胞损害,其保护机制可能与下调肝组织i NOS表达有关。
Objective To investigate the effects of hepatic ischemia reperfusion injury and its associated mechanism caused by the Danchengqi Decoction. Methods 24 SD rats were divided at random to three groups,Sham-operated group, Ischemia reperfusion group(IR group), and the Danchengqi Decoction pretreatment group. The animal models for severe hemorrhagic shock and resuscitation were replicated to keep their blood pressure at 5.3 k Pa(40 mm Hg), lasting90 min. After finishing with establishment of the animal models, histopathological changes were observed in animals of each group, and immunohistochemical assay was employed to detect expression and distribution of i NOS in the liver cells. Results Compared with sham group, in the IR group, i NOS expression was up-regulated in the hepatic tissue(P〈0.01). The hepatic cells were swelled,the hepatic sinusoid were narrowed and even occlusion,the inflammatory cells infiltrated,and patchy necrosis occurred. While compared with IR group, i NOS expression dropped dramatically(P〈0.01).The hepatic histopathological changes were improved. Conclution Dachenqi Decoction could alleviate cellular damage in the hepatic tissue caused by hemorrhagic shock and reperfusion injury in rats, the mechanism may be associated with inhibition of i NOS / NO expression.
