BAG-1蛋白、CA242与CEA联合检测对结直肠癌的诊断价值 被引量:7

Evaluation of combined test of BAG-1 protein, CA242 and CEA in diagnosis of colorectal cancer

摘要 目的探讨联合检测凋亡调控基因-1(BAG-1)蛋白、糖链抗原242(CA242)与癌胚抗原(CEA)肿瘤标志物在结直肠癌诊断中的应用。方法选取深圳市宝安区中心医院2013年1月—2014年12月收治的结直肠癌患者80例为研究组(按TNM分期分组:I期10例为I组,Ⅱ期30例为Ⅱ组,Ⅲ期32例为Ⅲ组,Ⅳ期8例为Ⅳ组),研究组患者均选取人结肠癌手术切除及化疗治疗,治疗有效54例为有效组,治疗无效26例为无效组,将同时期其他结直肠患者100例作为对照组,检测各组血清中BAG-1蛋白、CA242与CEA肿瘤标志物含量,并对各组检测结果进行比较分析。结果治疗前,研究组BAG-1蛋白、CA242与CEA肿瘤标志物含量分别为(52.96±16.75)U/m L、(228.90±76.31)U/m L和(49.08±5.71)ng/m L,对照组分别为(8.56±2.41)U/m L、(7.32±4.31)U/m L和(2.44±0.51)ng/m L,研究组明显高于对照组,差异有统计学意义(P<0.05);研究组结肠癌分期患者血清BAG-1蛋白、CA242与CEA肿瘤标志物检测之间差异显著,I组<Ⅱ组<Ⅲ组<Ⅳ组,TNM分期越高,含量越高,差异有统计学意义(P<0.05);血清BAG-1蛋白、CA242与CEA肿瘤标志物单项结果检测每个期检测结果均低于联合检测结果,差异有统计学意义(P<0.05);治疗后,有效组BAG-1蛋白、CA242与CEA肿瘤标志物含量明显低于无效组,差异有统计学意义(P<0.05)。结论联合检测BAG-1蛋白、CA242与CEA肿瘤标志物能提高结直肠癌诊断的准确率,对结直肠癌的辅助诊断有较高的临床应用价值。 Objective To evaluate the combined test of tumor markers, including apoptosis regulator gene-1(BAG-1)protein, carbohydrate antigen 242(CA242) and carcinoembryonic antigen(CEA), in the diagnosis of colorectal cancer.Methods The study group included 80 colorectal cancer patients treated in Bao'an District Central Hospital between January2013 and December 2014. Based on TNM classification, the study patients were further divided into four groups with 10 stage Ipatients in group I, 30 stage Ⅱcases in group Ⅱ, 32 stage Ⅲ patients in group Ⅲ; and 8 stage IV patients in group Ⅳ. Surgeryand chemotherapy were applied to all patients, and showed effective in 54 patients, and ineffective in the other 26 patients.Meanwhile, 100 patients with benign colorectal disorders were selected into the control group. Levels of serum BAG-1 protein,CA242 and CEA were examined and compared among groups. Results Before treatment, the study group showedsignificantly higher levels of BAG-1 protein, CA242 and CEA than control group [BAG-1 protein,(52.96 ± 16.75)U/m L vs.(8.56±2.41)U/m L; CA242,(228.90 ± 76.31)U/m L vs.(7.32 ± 4.31)U/ml; CEA,(49.08 ± 5.71)ng/m L vs.(2.44 ± 0.51)ng/m L; P〈0.05 respectively]. Concentrations of serum BAG- 1 protein, CA242 and CEA differed significantly among patientswith different colon cancer staging with the highest levels in group IV followed sequentially by group Ⅲ, II and I(P〈0.05respectively). The higher the TNM stage was, the greater the concentrations were. Single test results of BAG-1 protein, CA242 and CEA for each stage group were significantly lower than the combined test(P〈0.05). After treatment, concentrations ofBAG-1 protein, CA242 and CEA in the effective group were significantly lower in comparison to those in the ineffective group(P〈0.05 respectively). Conclusion Combined test of BAG- 1 protein, CA242 and CEA can improve the sensitivity indetection and accuracy in diagnosis of colorectal cancer, and thus provides a high value in diagnosis of colorectal cancer inclinical practice.
