摘要
1例76岁男性患者,既往支气管扩张病史20余年,否认既往药品不良反应史,此次因支气管扩张伴感染入院,入院后给予止咳平喘、祛痰等对症治疗,静脉滴注乳酸左氧氟沙星氯化钠注射液(0.5 g,q 24 h)抗感染,治疗第2天,患者于输液后3 h出现视物模糊,伴双踝关节轻度疼痛、肿胀,未予特殊处理。第3天静脉输液后约1.5 h再次出现视物模糊加重,视觉灰色,且伴有双踝关节肿痛症状加重,并累及双膝关节,及时停用乳酸左氧氟沙星氯化钠注射液,换用注射用哌拉西林他唑巴坦钠(4.5 g,q 8 h)静脉滴注抗感染并予关节局部热敷、理疗等对症治疗,1 d后患者视物模糊及视觉异常改善明显,停药9 d关节肿痛症状逐渐好转。
One 76-year-old male patient with past history of bronchiectasis for more than 20 years had no history ofadverse drug reactions. The patient was admitted to hospital due to the bronchiectasis and infection. After admission, antitussive,antiasthmatic and expectorant was administered for symptomatic treatment, and intravenous infusion of levofloxacin （0.5 g, q 24 h）was administered for anti-infection. On the second day, the patient appeared blurred vision, mild pain and swelling of double anklesafter infusion of levofloxacin for 3 hours, but got no special treatment. On the third day, the patient appeared blurred vision, visualgray, painful swelling of double ankles and knees after infusion of levofloxacin for 1.5 hours. Levofloxacin was stopped in time, thepatient received intravenous piperacillin and tazobactam sodium （4.5 g, q 8 h）, and symptomatic treatments such as anti-infection,joint local hot compress and physical therapy. One day later, the blurred vision and visual abnormalities improved obviously. After 9days accompanied drug withdrawal, the symptom of joints swelling pain gradually improved.
