血清sST2在评估心力衰竭严重程度及近期预后中的价值 被引量:13

Value of serum sST2 in evaluating severity and short-term prognosis of patients with heart failure

摘要 目的探讨血清可溶性ST2(sST2)水平在判断心力衰竭严重程度及近期预后中的价值。方法采用ELISA法测定206例急性心力衰竭或慢性心力衰竭急性发作患者血清sST2和血浆N末端B型脑钠肽前体(NT-proBNP)的表达。结果与心功能分级Ⅱ级患者相比,Ⅲ级、Ⅳ级患者血清sST2和血浆NT-proBNP水平均增高(P<0.05),Ⅳ级患者两者表达水平亦高于Ⅲ级患者(P<0.05)。心力衰竭患者血清sST2与血浆NT-proBNP水平呈正相关(r=0.33,P<0.01)。取血清sST2表达49.82ng/ml为临界值,其预测短期预后不良的敏感度为61.10%,特异度为82.10%。结论急性心力衰竭或慢性心力衰竭急性发作患者血清sST2水平明显升高;其高表达与心力衰竭严重程度显著相关,可作为评估短期预后的新生化指标。 Objective To investigate the value of serum soluble ST2(sST2)in evaluating the severity and short-term prognosis of patients with heart failure.Methods Serum expressions of sST2 and N-terminal pro-B-type natriuretic peptide(NT-proBNP)were detected by ELISA in 206 patients with acute heart failure(AHF)or chronic heart failure(CHF)with acute attack.Results Serum expressions of sST2 and NT-proBNP were significantly higher in the patients with NYHA Ⅲ and Ⅳ than those in those with NYHAⅡ(P〈0.05),which were also significantly higher in NYHA Ⅳthan those in NYHA Ⅲ patients(P〈0.05).Serum sST2 level was positively correlated with plasma NT-proBNP in the patients with heart failure(r=0.33,P〈0.01).Taking serum sST2 49.82ng/ml as a cutoff value,the sensitivity was 61.10% and the specificity was 82.10%in predicting apoor shortterm prognosis.Conclusion Serum sST2 expression is significantly increased in the patients with AHF or CHF with acute attack,which is associated with the severity of heart failure and can be used as a new indicator in predicting the short-term prognosis.
