摘要
目的探讨两种不同方法(椎弓根内固定和椎体成形术)在治疗老年胸腰椎压缩性骨折患者方面的临床疗效。方法病例来源于2010-10-2014-7我院骨科收治确诊的146例老年胸腰椎压缩性骨折患者,依据随机数字表法将两组患者均分成观察组和对照组各73例,其中给予观察组椎体成形术治疗,对照组给予椎弓根内固定手术治疗,比较两组患者的手术前后疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)评分等指标。结果观察组手术前后VAS评分、骨折椎体压缩率和ODI指数评分分别与对照组比较,差异无统计学意义(P>0.05);两组病患术后1周VAS评分、骨折椎体压缩率和ODI指数评分分别与本组术前比较,差异有统计学意义(P<0.01)。观察组术中出血量、手术时间、住院时间均明显优于对照组,差异有统计学意义(P<0.01)。结论椎体成形术治疗老年胸腰椎压缩性骨折可迅速缓解疼痛、减少病患手术损伤,促进病患健康恢复,安全可靠。
Objective To study the clinical curative effect of two different methods(pedicle internal fixation and percutaneous vertebroplasty) in the treatment of elderly patients with thoracic compression fractures. Methods There were 146 elderly patients diagnosed with thoracic compression fractures,from October 2012 to July 2014,who have treated in our hospital. According to random number table method,the patients were divided into observation group and control group,73 cases in each group. The observation group received the treatment of percutaneous vertebroplasty, the control group was given pedicle internal fixation surgery. Before and after surgery,pain visual analogue scale(VAS) and Oswestry disability index(ODI) score,etc were observed and compared in two groups. Results Before operation, VAS score,fracture vertebral compression rate and index of the ODI score in observation group had no obvious differences compared with control group(P〉0.05). The postoperative 1 week VAS score,fracture vertebral compression rate and index of the ODI score in two groups had significant differences compared with preoperation(P〈0.01). The intraoperative blood loss, operative time, length of hospital stay of the observation group were significantly better than the control group, the differences were statistically significant(P〈0.01). Conclusion The elderly percutaneous vertebroplasty compression fractures can rapidly relieve pain,reduce patient surgery injury,promote the patients health,and it is safe and reliabe.
