高频振荡通气治疗急性呼吸窘迫综合症的临床疗效 被引量:3

摘要 目的探讨高频振荡通气(HFOV)治疗急性呼吸窘迫综合症(ARDS)的疗效及安全性。方法将37例ARDS患者接受HFOV通气治疗,24例患者接受常频机械通气(CMV)治疗,动态监测两组患者2、12、24、48 h的p H值、血氧分压(Pa O2)、血二氧化碳分压(Pa CO2)、吸入氧浓度(FIO2)、改良氧合指数(OI)的变化以及中心静脉压(CVP),平均动脉动脉压(MAP)等指标的变化。结果两组上机前(0 h)的p H值、Pa O2、Pa CO2、FIO2、OI、CVP、MAP差异均无统计学意义(P>0.05),上机后2、12、24、48 h,p H值、Pa O2、Pa CO2、FIO2、OI差异均有统计学意义(P<0.05)。两组CVP、MAP差异无统计学意义(P>0.05)。结论高频振荡通气能显著改善ARDS患者的氧和,不会导致CO2的储留,对血流动力学干扰少,是治疗ARDS安全、有效的方法。
