生脉注射液对心脏瓣膜置换术丙泊酚全麻患者的心肌保护作用 被引量:9

Protective Effects of Shengmai Injection(生脉注射液) on Myocardium of Patients with Heart Valve Replacement under General Anesthesia with Propofol

摘要 目的探讨生脉注射液对丙泊酚全麻下心脏瓣膜置换术患者的心肌保护作用及可能机制。方法将40例心脏瓣膜置换术患者随机分为观察组和对照组各20例,观察组在丙泊酚麻醉前静脉注射生脉注射液1 ml/kg,对照组麻醉前静脉注射相同体积的生理盐水。比较两组患者进入手术室前、心肺转流前、心肺转流后、手术结束时的平均动脉压、心率、中心静脉压、血氧饱和度,检测静脉血中的丙二醛、超氧化物歧化酶和一氧化氮合酶的含量。结果观察组患者心肺转流前、后及手术结束时的平均动脉压显著高于对照组(P<0.05);观察组患者手术结束时心率显著高于对照组(P<0.05);观察组患者心肺流转前、后血丙二醛、一氧化氮合酶含量低于对照组,超氧化物歧化酶含量高于对照组(P<0.05)。结论生脉注射液对心脏瓣膜置换术丙泊酚全麻患者的心肌具有保护功能,增加心肌抗氧化物、减少氧化物是其可能的作用机制。 Objective To study the protective effects and possible mechanism of Shengmai Injection on myocardium of patients with heart valve replacement under general anesthesia with propofol. Methods Forty patients with heart valve replacement were randomly divided into the observation group and the control group,with 20 cases in each group. The observation group was given intravenous injection with Shengmai Injection( 1 ml / kg) before anesthesia with propofol. At the same time,the control group was given intravenous injection with the same volume of normal saline. The mean arterial pressure,heart rate,central venous pressure,blood oxygen saturation in two groups were compared,and the levels of malondialdehyde,superoxide dismutase,and nitric oxide synthase in venous blood were tested before entering the operating room,before and after cardiopulmonary bypass,and at the end of the operation.Results Before and after cardiopulmonary bypass and at the end of the operation,the mean arterial pressure was significantly higher in the observation group than that in the control group( P〈0. 05); At the end of the operation,heart rate in the observation group was significantly higher than that in the control group( P〈0. 05); Before and after cardiopulmonary bypass,levels of malondialdehyde and nitric oxide synthase were lower,but level of superoxide dismutase was higher in the observation group than those in the control group( P〈0. 05). Conclusion Shengmai Injection can protect myocardium of patients with heart valve replacement under general anesthesia with propofol. Its possible mechanism is due to increasing myocardial antioxidant and reducing oxide.
