摘要
目的探讨大剂量常规输血及去白细胞输血的临床效果。方法整群选取2013年7月—2014年7月在该院进行大剂量输血的88例患者,随机分成对照组和观察组,各44例。进行大剂量输血的88例患者随机分成对照组和观察组。对照组行常规输血,观察组行去白细胞输血,比较两组患者输血前后的临床效果。结果观察组不良反应率为4.55%,明显低于对照组的18.18%,且P<0.05;输血前后,观察组血清CD4+、CD8+、CD4+/CD8+无明显变化,而对照组血清CD4+、CD4+/CD8+明显降低;输血前后,观察组血清Ig G、Ig A、Ig M、Ig E无明显变化,而对照组输血后血清Ig G、Ig A、Ig M、Ig E降低明显。结论大剂量常规输血及去白细胞输血均能降低输血后的不良反应,但去白细胞输血效果更为显著,有助于患者的脏器功能够明显改善,值得推广。
Objective To investigate the clinical effect of large volume conventional blood transfusion versus leukoreduction transfusion. Methods 88 cases underwent large volume transfusion in our hospital from July 2013 to July 2014 were randomly divided into the control group and the observation group with 44 cases in each. The control group underwent the conventional blood transfusion, while the observation group underwent leukoreduction transfusion. And the clinical results were compared before and after transfusion. Results The incidence of adverse observations was much lower in the observation group than in the control group(4.55% vs 18.18%), P0.05. The difference in the serum CD4+, CD8+, CD4+/CD8+levels was not statistically significant in the observation group before and after transfusion. However, the serum CD4+, CD4+/CD8+levels decreased significantly in the control group after transfusion compared with those before transfusion. The difference in the serum Ig G, Ig A, Ig M and Ig E levels was not statistically significant in the observation group before and after transfusion,whereas the Ig G, Ig A, Ig M and Ig E levels decreased significantly in the control group after transfusion compared with those before transfusion. Conclusion Both large volume conventional transfusion and leukoreduction transfusion can reduce the incidence of adverse reactions, but the effect of the latter is more significant and conducive to the improving the organ function, so leukoreduction transfusion is worthy of promotion.
