螺旋CT及MRI检查在肝脏局灶性结节增生诊断中的价值研究 被引量:38

摘要 目的探讨肝脏局灶性结节增生(focal nodular hyperplasia,FNH)螺旋CT及MRI检查的影像学特点,以期提高FNH的诊断准确率。方法选取2011年5月—2013年2月经手术病理证实的36例FNH,术前根据影像学检查方法的不同分为螺旋CT组14例、MRI组12例及螺旋CT联合MRI组10例,分别评价其影像学特征和诊断准确率。结果螺旋CT组共有15个病灶,平扫示10个病灶呈低密度,增强扫描动脉期见病灶均明显强化,静脉期和延迟期有4个病灶减至等密度;诊断灵敏度为78.6%(11/14),特异度为85.7%(12/14)。MRI组共有13个病灶,T1WI序列示8个病灶呈低信号,T2WI序列示8个病灶呈混合高信号,增强扫描动脉期病灶均呈高信号,而静脉期和延迟期病灶强化呈减弱趋势;诊断灵敏度为83.3%(10/12),特异度为100.0%;螺旋CT联合MRI组诊断灵敏度、特异度均达100.0%。结论螺旋CT联合MRI检查可以明显提高FNH的诊断准确率。 Objective To explore the characteristics of imaging findings of spiral CT and MRI examination in diagnosing hepatic focal nodular hyperplasia( FNH),and improve the diagnostic accuracy rate of FNH. Methods 36 FNH cases confirmed by pathology during May 2011 and February 2013 were selected and were divided into spiral CT group( n = 14),MRI diagnosis group( n = 12) and spiral CT plus MRI group( n = 10),on the basis of different image logical examinations.Then their image logical features and diagnostic accuracy rate were evaluated separately. Results A total of 15 lesions in 14 cases in spiral CT scan group were scanned and showed that 10 FNH lesions were low density; enhanced scan showed that arterial lesions were obviously enhanced; four lesions venous phase and delayed phase were reduced to equal density; the sensitivity of FNH diagnosis was 78. 6%( 11 /14),and the specificity was 85. 7%( 12 /14). A total of 13 lesions in 12 patients with MRI scan group were scanned and showed that; there were eight lesions of T1 WI sequence showing low signal and eight lesions of T2 WI sequence showing mixed high signal; enhanced scan showed that arterial lesions were high signal; the venous phase and delayed phase showed a decreasing trend; the sensitivity of diagnosis FNH was 83. 3%( 10 /12),the specificity was 100. 0%. In spiral CT plus MRI group,both the sensitivity and specificity of diagnosis FNH were 100. 0%. Conclusion The joint use of spiral CT and MRI examinations can remarkably improve diagnostic accuracy rate,but because the sample is few,we still need more samples to confirm this prospective study.
