丙泊酚静脉全麻用于小儿MRI检查的安全性和有效性 被引量:7

摘要 目的探讨丙泊酚静脉全麻用于小儿MRI检查的安全性和有效性。方法选择2015年1~5月期间我院收治的需行MRI检查的患儿共60例随机分A、B、C组,每组20例,A组用丙泊酚2mg/(kg·h)持续静注,B组用丙泊酚4mg/(kg·h)持续静注,C组用丙泊酚6mg/(kg·h)持续静注。选择2015年5~10月期间我院收治的需行MRI检查的患儿共40例,随机分为A1组和B1组,A1组为丙泊酚静脉全麻组,B1组为水合氯醛灌肠组。结果 (1)B、C组完全合作率高于A组,基本合作率低于A组(P〈0.05);B组完全合作率、基本合作率与C组比较差异无统计学意义(P〉0.05)。三组检查时HR、MAP、Sp O2与给药前无统计学意义(P〉0.05),B组不良反应发生率低于A、C组(P〈0.05)。(2)丙泊酚静脉全麻组完全合作率高于水合氯醛灌肠组,基本合作率、不良反应发生率低于水合氯醛灌肠组(P〈0.05);水合氯醛灌肠组检查时HR、MAP、Sp O2与丙泊酚静脉全麻组比较波动程度更大(P〈0.05)。结论丙泊酚静脉全麻在小儿MRI检查中镇静效果更好,明显优于水合氯醛灌肠的效果,检查时的各项生理指标比较平稳,安全性更好,最佳剂量为4mg/(kg·h),可以在临床推广应用。 Objective To investigate the pediatric safety and efficacy of MRI with propofol intravenous anesthesia for pediatric. Methods 60 children for MRI examination in our hospital were selected from January 2015 to May 2015, they were randomly divided into group A, group B, group C, there were 20 cases in each group, group A treated with propofol 2mg/(kg·h) continuous intravenous, group B treated with propofol 4mg /(kg·h) continuous intravenous, group C treated with 6mg/kg.h continuous intravenous propofol. 40 children for MRI examination in our hospital were selected from May 2015 to October 2015, they were randomly divided into group A1(propofol intravenous anesthesia group) and group B1(hydrated chloral enema group). Results(1) The fully cooperation rate of group B, group C were higher than group A, basic cooperation rate were lower than group A(P〈0.05); there were no significant difference in complete cooperation rate, basic cooperation rate between group B and group C(P〈0.05). There were no significant difference in HR, MAP, Sp O2 between check moment and former administration(P〈0.05), The adverse reactions of group B were lower than group A, group C(P〈0.05).(2) The fully cooperation rate of propofol intravenous anesthesia group was higher than chloral hydrate enema group, the basic cooperation rate, incidence of adverse reactions was lower than chloral hydrate enema group(P〈0.05); The degree of fluctuation of chloral hydrate enema examination HR, MAP, Sp O2 was greater than propofol intravenous anesthesia group(P0.05). Conclusion Propofol intravenous anesthesia in pediatric sedation MRI examination is better, much better than the effect of chloral hydrate enema, examination of the physiological index is relatively stable, better security, the optimal dose of 4mg /(kg·h), can be applied in clinical practice.
