血栓弹力图与常规凝血检测指导临床输血的相关性分析 被引量:29

摘要 目的:探讨血栓弹力图(thromboelastography,TEG)与常规凝血试验的相关性,评价两种方法在指导临床合理用血中的差异。方法:选择临床医生申请进行TEG检测的住院患者72例,同时进行TEG检测和常规凝血项检测及血小板计数(PLTs),将所得TEG参数与常规凝血项指标进行相关性分析,并对两种方法指导的临床用血情况进行比较。结果:表示凝血因子的各参数比较,R值与APTT、PT呈正相关(P<0.05);表示纤维蛋白原和血小板的各参数比较,K值与FIB和PLTs呈负相关(P<0.05),Angle、MA与FIB和PLTs均呈正相关(P<0.05)。与常规凝血试验比较,TEG在指导临床用血中减少了输血率及各种血液制品的使用量(P<0.05)。结论:血栓弹力图参数与常规凝血试验指标存在明显相关性,血栓弹力图指导临床合理输血更具有指导意义,在临床实际应用中可将两者相互结合,起到互补的功能。 Objective:To investigate the correlations between the thromboelastogram(TEG) and conventional coagulation test,and compare the 2 methods' differences in guiding the reasonable clinical blood transfusion. Method:A total of 72 patients in hospital with TEG were enrolled,TEG,routine coagulation assaysand platelet count were detected simultaneously,the parameters of test results were made analysis for the relevant and compare the situation of two methods in guiding clinical blood transfusion.Result:Comparision of the clotting factor parameters,the value of R was positively correlated with APTT and PT(P〈0.05).Comparision of fibrinogen and platelet parameters,the value of K was negatively correlated with FIB and PLTs(P〈0.05),Angle and MA were positively correlated with FIB and PLTs(P〈0.05).Rate of blood transfusion and usage quantity of various kinds of blood products significantly reduced under the guidance of TEG test compared with traditional coagulation(P〈0.05). Conclusion:There are obvious Correlation between TEG parameters and those of routine haemostatic assays, TEG has more important significance to guide blood transfusion,TEG and conventional coagulation tests could be combined in clinical practice and had mutual complementary effect.
