原发性痛经患者大脑皮质形态学变化的观察 被引量:7

摘要 目的 研究与原发性痛经相关的大脑皮质形态上的变化,同时探讨此种变化和原发性痛经之间可能关系.方法 选取2015年1至12月,在同济大学附属同济医院妇产科门急诊就诊的20例原发性痛经的患者及20例年龄及月经情况与之类似正常对照,行头颅高分辨率结构像的磁共振扫描,扫描后利用SPM8对两组的全脑灰质体积进行基于体素的统计学处理.结果 与对照组相比,痛经组在左、右侧海马旁回;左、右侧颞中回;左侧顶上小叶与顶下小叶;左侧中央后回;右侧顶上小时与顶下小叶等脑区灰质体积萎缩变薄.结论 原发性痛经患者的部分脑区灰质确实存在萎缩变薄,这些脑区功能均与大脑疼痛处理和躯体感觉有关,提示这些变化与原发性痛经相关. Objective To investigate the structural changes of gray matter in women with primary dysmenorrhea (PDM) and discuss the potential relationships between the changes and etiology of the disease.Methods We used an optimized voxel-based morphometry (VBM) approach to compare total and regional volumetric changes of gray matter in 20 primary dysmenorrhea patients with 20 healthy age and menstrual cycle matched controls.All subjects were patients from the outpatient department of Tongji Hospital in 2015.Results Abnormal volumetric decreases of gray matter were found in the left and right parahippocampus,the left and right middle temporal gyms,the left and right superior parietal lobule,the left postcentral cortex,the left and right inferior parietal lobule,which involved in pain transmission,pain modulation and somatic sensation.Conclusion Abnormal gray matter volume changes are present in some encephalic regions of PDM patients.The functions of these regions are associated with pain modulation and somatic sensation,which suggests the changes are related with PDM.
