摘要
目的探讨血清胱抑素C（CysC）与血清视黄醇结合蛋白（RBP）检测在儿童狼疮性肾功能衰竭诊断中的临床意义。方法本研究将70例狼疮性肾衰竭患儿根据内生肌酐清除率（Ccr）的不同将患儿分成4个组别：I组（35例）：Ccr在50～80m L/min,肾功能代偿期;II组（15例）：Ccr在20～50m L/min,肾功能失代偿期;III组（10例）：Ccr在10～20m L/min,肾功能衰竭期;IV组（10例）：Ccr〈10m L/min,尿毒症期。检测血清CysC与血清RBP的水平（免疫速率散射比浊法）。同时,用酶法检测血清尿素氮（BUN）、血肌酐（Scr）。结果狼疮性肾衰竭患儿中,I组：CysC阳性率74.3%,RBP阳性率71.6%,Scr阳性率33.6%,BUN4.3%;II组：CysC阳性率91.1%,RBP阳性率88.3%,Scr阳性率50,2%,BUN37.8%;III组：CysC阳性率100%,RBP阳性率100%,Scr阳性率100%,BUN93.3%;IV组：Cys C异常率100%,RBP阳性率100%,Scr阳性率100%,BUN100%。血清胱抑素C、血清视黄醇结合蛋白与血清尿素氮、血肌酐呈正相关。血清胱抑素C的ROC曲线下面积（AUC=0.936）,血清视黄醇结合蛋白的ROC曲线下面积（AUC=0.928）均比血清尿素氮（AUC=0.715）和血肌酐（AUC=0.732）高（P〈0.05）。结论 CysC与RBP是狼疮患儿肾功能损伤的早期敏感指标,同时为狼疮性肾衰患儿肾功能损伤程度的判断提供一定的依据。
Objective To investigate diagnostic significance of serum cystatin C（ Cys） and serum retinol binding protein（ RBP） detection in children with lupus renal failure diagnosis. Methods A total of 70 patients with lupus renal failure were divided into 4 groups： Group I,35 casescreatinine clearance rate（ CCR） was at 50 ~ 80 m L /min. Group II,15 cases,CCR was at 20 ~ 50 m L / min. Group III,10 cases Ccr 10 m L / min group（ 10 cases）,IV group（ 20 cases）,50 m L / min group（ cases）,20 m L / min group（ cases）,Ccr group（ cases）. Detection of serum Cys C and serum RBP levels（ Immune rate scatter ratio）. At the same time,serum urea nitrogen（ BUN） and serum creatinine（ Scr） were detected by enzymatic method. Results I positive rate was,CysC positive rate was74. 3%,RBP positive rate was 71. 6%,Scr positive rate was 33. 6%,II positive rate was 91. 1%,RBP positive rate was 88. 3%,Scr positive rate was,III positive rate was 100%,RBP positive rate was 100%,BUN 93. 3%positive rate was 100%,IV positive rate was 100%,RBP positive rate was 100%,Scr positive rate was 100%,Cys C positive rate was,BUN100%. Serum cystatin C,serum retinol binding protein and serum urea nitrogen,serum creatinine positively. Serum cystatin C under the ROC curve area（ AUC = 0. 936） and serum retinol binding protein of the ROC curve area of AUC = 0. 928 were blood creatinine and serum urea nitrogen（ AUC =0. 715） AUC = 0. 732 high（ P〈0. 05） under. Conclusion Cys C and RBP are sensitive indicators of renal function damage in children with lupus,and provide a basis for judging the degree of renal damage in patients with renal failure.
