摘要
自身免疫性甲状腺疾病以自身免疫反应和甲状腺淋巴细胞浸润为特征,是引起甲状腺组织结构和功能异常的主要病因,除了能够导致甲亢或甲减及甲状腺结节和甲状腺肿大以外,还可导致甲状腺以外其他组织、器官和系统的损害,如甲状腺相关眼病、白细胞减少、肝脏损害、肌肉病变、女性月经不调及对生殖健康的影响,其原因是自身免疫紊乱。西药控制甲状腺疾病能够达到满意的疗效,但是对于甲状腺组织结构的改变以及甲状腺外相关症状,则效果欠佳。中医学称本病为瘿病,其病理产物(如沉积在甲状腺、白细胞等靶细胞上的免疫复合物以及甲状腺眼病眼局部产生过多细胞外物质等)主要由淋巴细胞浸润、免疫损伤引起,属中医痰瘀、血瘀范畴,故病机关键是气滞、痰凝、血瘀,治法为行气活血、祛痰化瘀。临床上通过西药将甲状腺功能控制正常后,再综合使用中医益气化瘀法,有利于缓解甲状腺组织结构改变以及由自身免疫紊乱导致的诸多症状,疗效满意。
Autoimmune thyroid disease characterized by autoimmune response and thyroid lymphocytic infiltration is a leading cause of the structure and function of thyroid tissue abnormalities. In addition to cause hyperthyroidism or hypothyroidism and thyroid nodules and thyroid enlargement, it can cause damage of other tissues, organs and systems,such as thyroid associated ophthalmopathy, leukopenia, liver damage, muscle lesions, menstrual disorders and affect reproductive health. The pathology is autoimmune disorders. Western medicine treatment of thyroid disease can achieve a satisfactory therapeutic effect, but the treatment effect is poor for the histological structure change of the thyroid gland and thyroid related symptoms. This disease is called as goiter by Chinese medicine, the pathological products（such as immune complexes deposited on target cells of thyroid, white blood cells and so on, and overmuch extracellular materials in local eyes caused by thyroid eye disease） are caused by lymphocyte invasion and immune injury, which belongs to the category of phlegm stagnation and blood stasis in traditional Chinese medicine, so the key pathogenesis is qi stagnation,coagulated phlegm, blood stasis, the basic treatment methods are promoting qi to activate blood, removing phlegm and blood stasis. In clinic, after the use of Western medicine in autoimmune thyroid disease to control thyroid function, the method of benefiting qi and removing blood stasis of traditional Chinese medicine is helpful to relieve the changes of thyroid tissue structures and many symptoms caused by autoimmune disorders, the curative effect is satisfactory.
