摘要
[目的]比较纤维支气管镜吸痰及传统吸痰方法在新生儿缺氧缺血性脑病(HIE)并发呼吸衰竭机械通气治疗中的差异,从而为临床吸痰护理方式的选择提供客观依据。[方法]选择2012年3月—2014年10月我院儿科重症监护室收治的HIE并发呼吸衰竭气管插管治疗的患儿60例,随机分为观察组30例和对照组30例,对照组采用常规方法进行吸痰,观察组采用纤维支气管镜吸痰,比较两组血气分析结果及黏膜损伤、肺内感染、肺不张发生率。[结果]观察组黏膜损伤发生率、肺内感染发生率、肺不张发生率低于常规吸痰法(P<0.05)。观察组第3天、第5天、第7天血气分析值优于对照组(P<0.05)。[结论]采用纤维支气管镜进行吸痰,吸痰效果更为确切,可明显提高血气分析中PaO2及PaO2/FiO2,减轻对气管支气管黏膜的破坏,降低肺内感染及肺不张发生率。
Objective:To compare the difference between the sputum suction by fibrous bronchoscopy and traditional sputum suction method applied for neonates with hypoxic-ischemic encephalopathy complicated with respiratory failure receiving mechanical ventilation,so as to provide an objective basis for the choice of clinical sputum suction nursing methods.Methods:A total of neonates with HIE complicated with respiratory failure in receiving trachea cannula were randomly divided into observation group and control group,30 cases in each,who came from the pediatric intensive care unit in our hospital from March 2012 to October 2014.The patients in control group received the routine sputum suction method,while the patients in observation group received the sputum suction by fibrous bronchoscopy,then to compare the blood gas analysis results,incidence of mucosal injury,incidences of pulmonary infection and pulmonary atelectasis between both groups.Results:The incidence of mucosal injury,incidences of pulmonary infection and pulmonary atelectasis in observation group were lower than that in control group(P<0.05).The blood gas analysis on the third days,fifth days,seventh days in observation group were better than those in control group(P<0.05).Conclusion:The suction effect of sputum suction by fibrous bronchoscopy was more precise and could significantly improve the PaO2,PaO2/FiO2 in blood gas analysis,relieve the bronchial mucosa damage,reduce the incidences of lung infection and pulmonary atelectasis,and it was the preferred sputum suction method in the nursing care of this kind of HIE neonates.
