摘要
目的探讨喉罩在老年腹腔镜胆囊切除术患者全身麻醉中的应用效果。方法将134例老年腹腔镜下胆囊切除术患者随机分为观察组与对照组,每组各67例。观察组患者采用喉罩机械通气,对照组患者采用气管插管机械通气。比较两组患者麻醉效果、血流动力学变化及围术期并发症发生率。结果两组患者手术顺利,术中麻醉满意,术后苏醒及时,状态良好。T0时间点两组患者收缩压(SBP)、舒张压(DBP)、心率(HR)及脉搏血氧饱和度(Sp O2)比较差异均无显著性(P>0.05)。与T0时间点比较,对照组患者T1时间点SBP、DBP及HR明显升高(P<0.01),SBP与HR在T2时间点显著降低(P<0.01);观察组患者T1时间点仅DBP明显升高,且升高水平显著低于对照组(P<0.01);T_1、T_4时间点观察组患者SBP、DBP及HR均显著低于对照组(P<0.01)。两组患者5个时间点Sp O_2均未发生显著变化(P>0.05)。观察组患者围术期并发症发生率均显著低于对照组(P<0.05或P<0.01)。结论喉罩在老年腹腔镜胆囊切除术患者全身麻醉中保证良好通气及麻醉状态的同时,使血流动力学更加稳定,咽喉部并发症明显减少,安全性较高,值得推广。
Objective To discuss the clinical effects of laryngeal mask airway general anesthesiain in elderly patients undergoing laparoccopic cholecystectomy. Method 134 elderly patients undergwent laparoccopic cholecystectomy were randomly divided into observation group and control group, 67 cases in each group. Observation group patients were given laryngeal mask airway general anesthesiain and control group patients were given intubation general anesthesiain. The anesthetic effects, hemodynamics, and perioperative complication rates were compared between the two groups. Result Anesthesia were satisfactory in the two groups. There were no significant differences in systolic blood pressure（SBP）, diastolic blood pressure（DBP）, heart rate（HR） and blood oxygen saturation（Sp O2） between the two groups at the time of T0. Compared with T0 time point, control group patients with T1 time point of SBP, DBP and HR increased significantly（P 0.01）, SBP and HR in T2 time point were significantly reduced（P 0.01）; observation group of patients with T1 time point only DBP increased significantly, and higher level was significantly lower than control group（P 0.01）. T1, T4 time point observation group of patients with SBP, DBP and HR were significantly lower than control group（P 0.01）; there were no significant changes of Sp O2 in the two groups at each time point（P 0.05）. The incidence of perioperative complications in observation group were significantly lower than control group（P 0.05 or P 0.01）. Conclusion Laryngeal mask airway general anesthesiain can ensure good ventilation and anesthesia, and at the same time get a more stable hemodynamic, and reduce throat local complications in elderly patients undergoing laparoccopic cholecystectomy, has a high security, and worthy of promotion.
