孕期亚临床甲减对患者后代智力影响及干预研究 被引量:5

INFLUENCE OF PREGNANCY SUBHYPOTHYROIDISM ON OFFSPRING INTELLEGENCE AND ITS INTERVENTION STUDY

摘要 目的探讨孕期亚临床甲减对患者后代智力的影响及干预研究。方法选择2010年1月—2014年6月在该院进行甲状腺功能检测的150例孕妇为观察对象。包括亚临床甲减组50例,50例自愿接受L-T4干预(SCH+LT4组)和正常妊娠对照组(正常组)50例。对亚临床甲减患者的后代在新生儿期、12、15、18月时的智力和运动;三组患者的母亲及其后代的甲状腺功能(FT3、FT4、TSH)进行分析研究。结果对各组母亲血清TSH、FT4、FT3水平进行分析,TSH、FT4在各组中的差异具有统计学意义(P<0.05)。SCH组和TSH+LT4组比较无统计学意义(P>0.05);对三组后代的FT3、FT4、TSH比较,新生儿期SCH组和SCH+LT4组的HST值为(5.30±1.65)、(5.31±1.66),均高于正常组的(4.43±1.21),差异有统计学意义(P<0.05)。在12个月、15个月、18个月时,三组后代的FT3、FT4、TSH值接近,差异无统计学意义(P>0.05);对三组孕妇后代的智力评分,正常组和SCH+LT4组的值均高于SCH组的值,具有统计学意义(P<0.05)。从后代的运动评分,SCH组值明显低于正常组和SCH+LT4组,具有可比性(P<0.05)。结论孕期妇女亚临床甲状腺功能减退症对后代智力和运动等其他机体功能造成不良影响,LT4对治疗妊娠早期亚临床甲减患者的TSH水平具有重要效果。 Objective To explore the effectof of subhypothyroidism for pregnancy on the intelligence of offspring,and study its intervening measures.Methods A total of 150 cases of pregnant women were selected as the observation subjects between January 2010 and June 2014 in 4~20 weeks of gestation who received thyroid function tests in author's hospital.In the study group,including subclinical hypothyroidis group of50 cases(TSH group),50 cases of voluntary L-T4 intervention group(SCH + LT4 group)and 50 cases of normal pregnancy control group.For the offspring of subhypothyroidism pregnant wome,the intelligence and movement were observed in the neonatal period,12,15,18 months,and the thyroid function(FT3,FT4,TSH)of mother and her offspring of three groups was analyzed.Results For mother serum thyroid function,the level of TSH,FT4 in three groups was the statistically significant difference(P〈0.05),but they were no statistical significance between the SCH and TSH + LT4 groups(P〉0.05).For the comparison of three groups of children,in the neonatal period of SCH group and SCH + LT4 group HST value were(5.30 ± 1.65),(5.31± 1.66),higher than in normal group(4.43± 1.21)with a significant difference(P〈0.05).In the 12 months,15 months and 18 months,FT3,FT4,TSH values of three groups of children were no statistical significance(P〉0.05).For intelligence scores of three groups of pregnant women offspring,the value of the normal group and the SCH+LT4 group were higher than that of SCH group value,with statistical significance(P〈0.05).The values from the offspring of sports scores,SCH group was obviously lower than normal group and SCH+LT4 group(P〈0.05).Conclusions Pregnant women with subclinical hypothyroidism can influence offspring intelligence and movement function.LT4 can play an important role in the treatment of patients with subclinical hypothyroidism on TSH level in early pregnancy.
