细胞间黏附分子1高表达与大鼠SAP早期急性肺损伤的关系

摘要 目的探讨细胞间黏附分子1(intercellular adhesion molecule-1,ICAM-1)高表达与大鼠重症急性胰腺炎(severe acute pancreatitis,SAP)早期急性肺损伤的关系。方法 40只健康成年雄性SD大鼠,依据随机数字表法分为研究组(n=30)和对照组(n=10)2组。将0.1mL/100g 4%牛黄胆酸钠匀速注入研究组大鼠胆胰管中,将等量生理盐水匀速注入对照组大鼠胆胰管中,然后对2组大鼠的血淀粉酶及血气变化、肺含水率、髓过氧化物酶(myeloperoxidase,MPO)活性、肺组织学评分及胰腺组织学评分、肺ICAM-1表达进行统计分析。结果研究组大鼠术后3、6、12h不同时点血淀粉酶和动脉血二氧化碳分压(partial pressure of carbon dioxide in arterial blood,PaCO2)水平、肺含水率、MPO活性、肺组织学评分和胰腺组织学评分、肺ICAM-1积分光密度值和面积均逐渐升高(P<0.05);氧合指数和PaO2水平均逐渐降低(P<0.05)。研究组大鼠术后12h血淀粉酶和PaCO2水平、肺含水率、MPO活性、肺组织学评分和胰腺组织学评分、肺ICAM-1的面积均显著高于对照组(P<0.01);氧合指数、PaO2水平及肺ICAM-1的积分光密度值均显著低于对照组(P<0.01)。结论 ICAM-1高表达极易引发大鼠SAP早期急性肺损伤。 Objective To investigate the relationship between the high expression of intercellular adhesion molecule-1(ICAM-1)and early acute lung injury of rats with severe acute pancreatitis.Methods Forty healthy adult male SD rats were divided into two groups according to a random number table:study group(n=30)and control group(n=10).The study group of rat were given 0.1 mL/100 g 4% sodium taurocholate uniformly injected through bile duct,while the control group of rats were given inject saline uniformly injected through bile duct,and then the blood amylase enzymes and blood gas changes,pulmonary water containing rate,myeloperoxidase(MPO)activity,lung and pancreatic histological scores,the expression of ICAM-1in lung of the two groups of rats were analyzed.Results In comparison with control groups,blood amylase,partial pressure of carbon dioxide in arterial blood(PaCO2)levels,pulmonary water containing rate,MPO activity,lung and pancreatic histological scores,lung ICAM-1integrated optical density values in the study group at 3h,6h,12 h were gradually increased(P〈0.05),the oxygenation index,PaO2 levels were decreased(P〈0.05);In comparison with control groups,the serum amylase,PaCO2 level,pulmonary water containing rate,MPO activity,lung and pancreatic histological scores and area of lung ICAM-1in the study group were significantly higher than that in control group(P〈0.01),However,the oxygenation index,PaO2,integrated optical density values were significantly reduced in study group(P〈0.01).Conclusion High expression of ICAM-1can easily lead to early acute lung injury of rats with severe acute pancreatitis.
