摘要
目的探讨老年2型糖尿病合并冠心病患者血清抵抗素与相关代谢指标的关系。方法选取2011年4月—2014年10月首都医科大学宣武医院收治老年2型糖尿病患者87例,分为单纯糖尿病组(单纯病组)45例和合并冠心病组(合并病组)42例,并选择同期老年健康体检者30例作为对照(健康对照组)。测定空腹血清抵抗素、空腹血糖(FPG)、餐后2小时血糖(2 hPG)、空腹胰岛素(FINS)和血脂水平[胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)];用稳态胰岛素评价指数(HOMA-IR)评价胰岛素抵抗的程度,分析各指标间的相关性。结果合并病组患者血清抵抗素>单纯病组>健康对照组[分别为(4.98±1.14)ng/ml、(4.02±0.76)ng/ml、(1.59±0.91)ng/ml,F=23.025,P<0.01],相关分析发现,血清抵抗素浓度与BMI、HOMA-IR呈显著正相关(r=0.578,P<0.05;r=0.792,P<0.05),与HDL-C呈负相关(r=-0.738,P<0.05)。结论老年2型糖尿病合并冠心病患者血清抵抗素水平明显升高,提示在糖尿病及糖尿病向大血管病变发展过程中,血清抵抗素可能发挥了一定作用。
Objective To investigate the relationship between serum resistin level and metabolic index in elderly type2 diabetes mellitus with coronary heart disease.Methods Select from 2011 April to October 2014,87 cases of elderly type 2diabetic patients in Beijing Xuanwu Hospital,Capital University of Medical Sciences were enrolled and divided into simple diabetes group（disease group,45 cases） and coronary heart disease group（hypertension group,42 cases）,and selected 30 cases of the same elderly health examination as control（healthy control group）.Determination of fasting serum resistin,fasting plasma glucose（FPG）,meal after 2 hour plasma glucose（2 hPG）,fasting insuhn（FINS） and lipid levels cholesterol（TC）,triglycerine（TG）,high density lipoprotein cholesterol（HDL-C）,low density lipoprotein cholesterol（LDL-C）;insulin homeostasis assessment index（HOMA-IR） evaluation of insulin resistance,analyze the correlation among the indexes.Results The serum resistin levels in patients with the order of simple group 〉disease group 〉healthy control group[respectively（4.98±1.14） ng/ml,（4.02±0.76） ng/ml,（1.59 ±0.91） ng/ml,F =23.025,P〈0.01].Correlation analysis showed that,serum resistin levels and BMI,HOMA-IR showed significant positive correlation（r =0.578,P〈0.05;r =0.792,P〈0.05）,and HDL C was negative correlated（r =-0.738,P〈0.05）.Conclusion Elderly type 2 diabetes mellitus complicated with coronary heart disease patient blood serum resistin level significantly increased,suggesting that in diabetes with the development process to vascular lesions,the serum resistin may play a certain role.
