摘要 目的:探讨PDCA循环管理法用于控制抗菌药物使用强度(AUD)的效果,促进抗菌药物合理应用。方法:运用PDCA循环理论进行现状分析、找出原因、确定目标、制定对策并实施干预。将我院实施PDCA循环前2013年1-6月与实施PDCA循环后2013年7月-2015年6月(每3个月为一个干预阶段)的抗菌药物使用情况进行比较分析。结果:经过8个PDCA循环干预阶段后,抗菌药物使用率由基线的51.52%下降至42.89%;使用合理率由65.55%上升至87.87%;微生物送检率由48.67%上升至88.24%。Ⅰ类切口抗菌药物预防使用率由基线的38.89%降至28.82%;术前30 min至2 h给药率由55.02%提高至94.62%;术后24至48 h停药率由51.67%提高至83.87%。抗菌药物使用强度由基线的50.75(DDD/100人/天)降至35.96(DDD/100人/天);与干预前基线值比较,均具有显著性差异(P<0.05)。结论:PDCA循环管理用于抗菌药物使用强度的控制及促进抗菌药物使用持续改进成效显著,可在抗菌药物管理中推广应用。 Objective: To evaluate the effect of PDCA circle management method on controlling antibiotics use density(AUD), so as to promote the proper applications of antibiotics. Methods: The PDCA circle theory was employed to analyze the status, to find out reasons, to set goals, to formulate strategies and to implement intervention. The use of antibiotics before the implementation of PDCA circle management in the hospital from January 2013 to June 2013 and after the implementation of PDCA circle management in the hospital from July 2013 to June 2015(every three months are regarded as an intervention period) was compared and analyzed. Results: After eight PDCA circle intervention periods, the utilization rate of antibiotics decreased from 51.52%to 42.89%; the use reasonability rose from 65.55% to 87.87%; the inspection rate of microorganism increased from 48.67% to 88.24%; the utilization rate of I-type incision antibacterial medicine prevention decreased from the baseline of 38.89% to 28.82%; the dose rate 30 minutes to 2 hours before the operation increased from 55.02%to 94.62%; the dose rate 24 hours to 48 hours after the operation increased from 51.67% to 83.87%; the AUD decreased from the baseline of 50.75(DDD/100 person/day) to 35.96(DDD/100 person/day); compared with the baseline value before intervention, the difference is significant(P0.05). Conclusion: PDCA circle management is effective in controlling AUD which is continuously improving the use of antibiotics. Thus, it can be widely applied to antibiotics management.
