摘要
目的研究血浆脂蛋白磷脂酶A_2（Lp-PLA_2）在脑梗死患者中的水平及意义。方法选取2015年1~12月期间在我院神经内科收治的120例动脉粥样硬化性脑梗死患者作为病例组,同时随机抽取同期我院正常体检者120例作为对照组,两组对象分别行Lp-PLA_2、同型半胱氨酸（Hcy）、低密度脂蛋白胆固醇（LDL-C）、纤维蛋白原（Fib）、空腹血糖（Glu）、血白细胞（WBC）等检测,比较两组间上述检测值的差异,同时病例组根据病情严重程度（以美国国立卫生研究院卒中量表评分作为标准）分为轻、中、重型脑梗死三组,比较三组间Lp-PLA_2水平的差异,并根据Spearman相关分析得出Lp-PLA_2与Hcy、LDL-C、Fib、Glu、WBC的相关性。结果病例组Lp-PLA_2、LDL-C、Hcy、Fib、WBC水平高于对照组,差异有统计学意义（P〈0.05）;其中Lp-PLA_2在轻、中、重型脑梗死三组中递增升高,与病情严重程度呈正相关,差异有统计学意义（P〈0.05）,Lp-PLA_2水平与LDL-C呈正相关,与Hcy、Fib、Glu、WBC无相关性。结论 Lp-PLA_2水平的高低反映脑梗死的严重程度,临床上可以通过检测Lp-PLA_2作为脑梗死预后一种新的预测方法,为临床治疗决策提供相关指导。
Objective To study the level and significance of plasma lipoprotein phospholipase A_2（Lp-PLA_2） in patients with cerebral infarction. Methods 120 patients with atherosclerotic cerebral infarction during January to December2015 in neurology department of our hospital were selected into case group,and 120 healthy persons during the same period were randomly selected from medical examination center of our hospital into control group.Lp-PLA_2,homocysteine（Hcy）,low density lipoprotein cholesterol（LDL-C）,fibrinogen（Fib）,fasting blood glucose（Glu）,and white blood cell（WBC） of the two groups were tested and compared.The subjects of cerebral infarction group were divided into mild,moderate and severe three groups,according to the severity of the disease（according to the National Institutes of Health Stroke Scale score）.The Lp-PLA_2 levels of the three groups were compared,and the correlations of Lp-PLA_2 with Hcy,LDL-C,Fib,Glu,and WBC were obtained according to the Spearman′ s correlation analysis. Results The levels of Lp-PLA_2,LDL-C,Hcy,Fib and WBC in the case group were higher than those in the control group respectively,with statistical significance（P〈0.05）.Lp-PLA_2 levels of the mild,moderate and severe groups of cerebral infarction were increased in turn,which were positively correlated with the severity of the disease,with statistical significance（P〈0.05）.The level of Lp-PLA_2 was positively correlated with LDL-C level,while it had no correlation with Hcy,Fib,Glu and WBC levels.Conclusion The level of Lp-PLA_2 reflects the severity of cerebral infarction,thus it can be used as a new method to predict the prognosis of cerebral infarction,and provide guidance information for clinical treatment decision.
