摘要
目的:总结泌尿内镜经耻骨上膀胱造瘘途径在等离子前列腺剜除术中的应用价值。方法:选取2009年1月-2015年12月在笔者所在医院采用经尿道等离子前列腺剜除术治疗的良性前列腺增生患者120例,术中采用泌尿内镜经耻骨上膀胱造瘘途径检查并处理膀胱内残留前列腺组织碎片、膀胱内结石及结石碎片、膀胱颈组织的完整性和出血等情况。结果:120例患者经上述方法顺利完成等离子剜除术,平均手术时间65 min,术后平均冲洗时间2 d,术后平均留置尿管时间5 d,术后平均住院时间6 d,术后无结石残留、无出血输血病例、无尿管滑脱等术后并发症,所有患者均经一期愈合治愈出院。结论:泌尿内镜经耻骨上膀胱造瘘途径在等离子前列腺剜除术中的应用,可以检查并处理残留前列腺组织、发现并处理膀胱内结石及残留的结石碎片、观察膀胱颈的完整性和处理膀胱颈盲区的出血情况、观察并调整导尿管气囊的体积,可以减少手术并发症,值得临床推广应用。
Objective： To summarize the application value of urinary endoscope through suprapubic cystostomy in plasma enucleation of the prostate.Method：120 patients with benign prostatic hyperplasia were treated by transurethral plasmakinetic resection from January 2009 to December 2015 in our hospital,urinary endoscope was used intraoperative after suprapubic bladder made fistula way inspected and treatment with the bladder residual prostate tissue fragments,bladder stones and stone debris, bladder neck tissue integrity and bleeding.Result：120 patients with the methods above were completed successfully. The average operation time was 65 minutes, the postoperative average flushing time for 2 days,postoperative average indwelling catheter time for 5 days,postoperative hospitalization time for 6 days,postoperative no residual stones,no bleeding blood transfusion,no catheter spondylolisthesis surgery complications,all patients were cured by primary healing.Conclusion：Urinary endoscope after suprapubic bladder made fistula way in plasmakinetic enucleation of prostate application can check and treat residual prostate tissue,discovery and treatment of bladder calculi and residual stone fragments,observation of bladder neck of integrity and for the treatment of bladder neck blind the blood,observe and adjust the guide catheter gasbag volume,can reduce the operative complications,it is worthy of clinical application.
