冲压法上颌窦底提升术用于上后牙区剩余牙槽骨高度不足5mm时种植修复的回顾性研究 被引量:6

Retrospective study of the osteotome sinus floor elevation for implantation restoration in the upper posterior area when residual bone height is less than 5 mm

摘要 目的评估上后牙区牙缺失后剩余牙槽骨高度不足5 mm时运用冲压法上颌窦底提升术进行种植修复是否安全可靠。方法选择2011年1月至2014年1月于第四军医大学口腔医院口腔种植科行种植修复的上颌后牙区缺牙后牙槽嵴严重萎缩(剩余牙槽骨高度不足5 mm)病例26例,均采用冲压法上颌窦底提升术植入植骨材料并同期植入种植体,收集患者影像学资料并测量种植体周软组织指数,进行回顾性研究。分别测量术后及最终修复体戴入1年后随访时的边缘骨水平、根尖骨高度及软组织指数并进行分析。结果共35颗种植体纳入本研究,边缘骨吸收为(1.21±0.40)mm,术后根尖骨高度为(1.33±0.52)mm,随访时根尖骨高度为(1.20±0.52)mm,随访时根尖骨高度较种植体植入后降低,且差异有统计学意义(P<0.05)。随访时35颗种植体的改良菌斑指数(mPLI)为1.46±0.78,改良出血指数(mSBI)为1.26±0.74。结论当上颌后牙区剩余牙槽骨高度不足5 mm时,可运用冲压法上颌窦底提升术植入种植体,其效果较为满意。 Objective To prove whether it is safe and reliable to use osteotome sinus floor elevation to implant when the residual bone height in posterior maxilla area is less than 5 mm, and with no tooth there. Methods Collect data and in- formation of 26 patients who planted and repaired the posterior maxilla area by using osteotome sinus floor elevation from January, 2011 to January, 2014 in our department whose residual bone height was less than 5 mm as well as the imaging data and index of soft tissue implant, and then do a retrospective study on these data. Take measurements of the crest bone level, the apical bone height as well as the soft tissue index at the time of postoperative and the one-year follow-up and analysis was performed. Results Totally 35 implants with an average length of follow-up of 12 months after the fi- nal restoration had been finished; marginal bone loss was (1.21±0.40)mm and apical bone height was(1.33±0.52) ram. The apical bone height was ( 1.20±0.52)ram during the follow-up visits, and it was reduced after the implants was implanted, and the difference had statistical significance. The index of soft tissue mPLI was 1.46±0.78 during the fol- low-up visits. The mSBI was 1.26±0.74. Conclusion Osteotome sinus floor elevation is proved to have satisfactory result in implantation when the residual bone height in posterior maxilla area is less than 5 mm.
