共聚焦激光扫描显微镜在不典型汗管瘤中的应用 被引量:3

摘要 目的探讨皮肤共聚焦激光扫描显微镜(CISM)诊断汗管瘤的应用价值。方法对24例临床诊断为扁平疣、脂溢性角化病但不排除汗管瘤的患者,选定典型皮损做CLSM检查,后取该处皮损行组织病理学检查。结果 CLSM图像显示表皮大致正常,真皮浅层或中层可见圆形、卵圆形的高折光性结构,组织病理检查结果提示表皮大致正常,真皮内可见多数导管、小的囊腔及由上皮细胞所组成的小细胞巢及索,这与CLSM图像下的高折光性结构一致。结论 CLSM是诊断汗管瘤及鉴别扁平疣、脂溢性角化病等皮肤病非常有用的辅助工具。 Objective To investigate the value of confocal laser scanning microscopy in the diagnosis of atypical syringoma. Methods Twenty-four cases that clinically diagnosed as fl at wart or seborrheic keratosis, but could not to exclude the diagnosis of syringoma, were enrolled in this study. The selected skin lesion of all the patients were examined by confocal laser scanning microscopy, and then by histopathologic examination. Results The image showed that the epidermis was roughly normal. There was round and ovoid high index structure in shallow or middle dermis, which was consistent with histopathological findings. Histopathology showed that the epidermis was normal, there were catheters, small cysts and small cell nests and cables composed by epithelial cells in the intradermal, which was consistent with the image of a high refractive structure. Conclusion CLSM is helpful to distinguish syringoma, fl at wart and seborrheic keratosis.
