胱抑素C在儿童急性肾损伤病情评估中的价值分析 被引量:1

摘要 目的探讨胱抑素C在儿童急性肾损伤病情中的评估价值。方法选取2012年3月~2015年1月收治的113例急性肾损伤患儿作为研究组,根据病情分为1期组、2期组、3期组,同时选取体检中心体检健康儿童50例作为对照组,分别检测并比较四组儿童胱抑素C(Cys C)、血肌酐(SCr)、血尿素氮(BUN)水平,并分析急性肾损伤患儿Cys C水平与SCr、BUN及慢性健康状况评分Ⅱ(APACHEⅡ)相关性。结果研究组患儿Cys C、SCr、BUN水平均显著高于对照组,且随着疾病分期增加而升高,比较差异有统计学意义(P〈0.001)。随着Cys C水平升高,研究组患儿SCr、BUN水平及APACHEⅡ评分逐级升高,比较差异有统计学意义(P〈0.001)。结论急性肾损伤患儿胱抑素C水平显著升高,且与患儿SCr、BUN水平及APACHEⅡ评分密切相关,可作为临床评估急性肾损伤患儿病情的指标。 Objective To evaluate clinical value of cystatin C in children with acute kidney injury.Methods 113 cases of children with acute kidney injury admitted to hospital from March, 2012 to January,2015 were selected as research group. All patients were divided into first stage group, second stage group and third stage group according to their conditions.At the same time,50 healthy children undergoing physical examination in the physical examination center were selected as control group.Then, cystatin C (CysC), serum ereatinine (SCr) and blood urea nitrogen (BUN) levels of the four groups were detec- ted and compared.And correlation between CysC levels and SCr, BUN and chronic health status score Ⅱ (APACHE Ⅱ) was analyzed.Results CysC, SCr and BUN levels of the research groups were significantly higher than those of the control group, and they increased with the increase of the stages of disease,so difference was statistically significant (P〈0.001).With the increase CysC level, SCr and BUN levels and APACHE Ⅱ scores of the research groups gradually increased, so the difference was statistically significant (P〈0.001).Conclusion Cystatin C levels of children with acute kidney injury significantly in- crease, and it is closely related with the SCr, BUN levels and APACHE Ⅱ score.Thus ,it can be used as detection index in the clinical assessment of children with acute renal injury.
