ASSOCIATION OF PAPP-A AND COX-2 WITH STABILITY OF CAROTID ATHEROSCLEROTIC PLAQUE

摘要 目的了解妊娠相关蛋白-A(PAPP-A)、环氧合酶-2(COX-2)在人颈动脉粥样硬化斑块中的表达情况,探讨COX-2、PAPP-A与斑块稳定性及颈动脉狭窄程度的关系。方法人颈动脉粥样硬化斑块46例,其中不稳定斑块(UP)36例,稳定斑块(SP)10例;中度狭窄9例,重度狭窄37例。应用细胞免疫组化方法,检测各斑块中COX-2、PAPP-A表达情况,分析两者表达的相关性。结果 UP组中PAPP-A和COX-2表达阳性细胞数均明显高于SP组,差异有显著性(t=16.83、15.88,P<0.01);UP组和SP组PAPP-A和COX-2表达均存在正相关关系(r=0.74、0.60,P<0.05)。中度狭窄斑块中PAPP-A和COX-2表达阳性细胞数与重度狭窄斑块比较,差异无统计学意义(P>0.05)。结论 COX-2、PAPP-A参与了颈动脉粥样硬化的形成和发展;COX-2、PAPP-A在斑块中的表达与颈动脉的狭窄程度无关;PAPP-A、COX-2对颈动脉粥样硬化斑块具有协同刺激作用。 Objective To investigate the expressions of pregnancy-associated plasma protein-A (PAPP-A) and cyclooxy- genase-2 (COX-2) in carotid atherosclerotic plaques (CAP), and explore their association with the stability of plaque and the de- gree of carotid artery stenosis (CAS). Methods Forty-six patients with CAP were enrolled, in which, 36 with unstable plaque (UP) and 10 with stable plaque (SP). Moderate stenosis was noted in nine cases, and severe stenosis in 37 cases. Using immuno- histochemistry, the expressions of COX-2 and PAPP-A in each plaque were detected, and the correlation between the two expres- sions was analyzed. Results In UP group, the numbers of positive expressions of PAPP-A and COX-2 cells were higher than that in SP group (t = 16.83,15.88;P〈0.01) the expression of PAPP-A was positively correlated with COX-2 in UP and SP groups (r=0.74,0.60 P〈0.05). The difference in the expressions of PAPP A and COX-2 between moderate stenosis plaque and severe stenosis plaque was not statistically significant (P〈0.05). Conclusion PAPP-A and COX-2 are involved in the formation and development of carotid atherosclerotic plaque. The expressions of PAPP-A and COX-2 are not associated with the degree of stenosis in carotid artery. PAPP-A and COX-2 play a synergistic stimulation on the plaque.
