摘要
目的对四孔法、三孔法以及单孔法腹腔镜下胆囊切除术对患者预后及生活质量的影响进行观察和分析。方法以普外科2012年6月-2015年6月收治的60例行腹腔镜胆囊切除术的胆囊炎、胆囊息肉、胆结石患者作为研究对象,对其临床资料进行分析,比较四孔法、三孔法及单孔法腹腔镜下胆囊切除术术中一般情况、患者术后恢复情况以及并发症的影响,并对其消化系统疾病生活质量指数进行比较分析。结果单孔组在在术中出血量、住院时间、住院费用方面较四孔组和三孔组均有明显下降,但在手术时间上则明显增加（P〈0.05）,差异具有统计学意义;术后结束治疗时单孔组患者其在生理功能状态、日常生活和社会活动状态、心理情绪状态以及生活质量总评分上均高于四孔组和三孔组（P〈0.05）,差异具有统计学意义。结论单孔腹腔镜下胆囊切除术的出现减少了患者术中出血量,缩短其住院治疗时间,进而有效的缓解了患者的经济压力,同时也有效提高了患者结束治疗时的生活质量,在最短的时间内恢复患者的正常工作和生活,对临床治疗具有积极的作用。
Objective To observe and analyze the effect of laparoscopic cholecystectomy by four holes,three holes and single hole for the prognosis after laparoscopic cholecystectomy and the quality of life of patients. Methods To choose 60 cases of the clinical data of patients with cholecystitis,cholecystic polypus or gall- stone who were treated by laparoscopic cholecystectomy in the general surgery department of our hospital from June 2012 to June 2015 as the research objects. To retrospectively analyze and compare general condition,the influence of postoperative recovery and complications,disease of digestive system and the quality of life index by the method of four hole,three hole and single hole of laparoscopic cholecystectomy in patients. Results The volume of the inoperative blood loss,hospital stay time,hospital costs of the single- hole group were significantly lower than that of the four hole group and the three hole group,but on the operation time showed significantly increased（ P〈0. 05）,the difference was statistically significant; The patients of the single- hole group in the state of physiological function,state of daily life and social activities,psychological emotional state and quality of life of the total score in the time of postoperative treatment towards the end were higher than that in the four hole group and the three hole groups（ P〈0. 05）,the difference was statistically significant. Conclusions The single- hole laparoscopic cholecystectomy can reduce the inoperative blood loss of patients,shorten their hospitalization time,thus effectively relieve the economic pressure of the patients,and it also effectively improve the treatment towards the end of the patient＇s quality of life,restore the patient＇s normal work and life in the shortest possible time,it plays an active role in clinical treatment.
