摘要
目的:评价目前补充替代疗法治疗腰椎间盘突出症的随机对照试验(RCT)报告。方法:检索了6个中英文数据库,时间范围设定为从数据库成立到2013年12月。纳入符合条件的随机对照试验报告的研究选择,数据提取和验证由2名试验人员分别进行。结果:从1990年到2012年间,有662项随机对照试验(88 129例)符合纳入标准,然而大部分试验方法质量偏低。其中43.11%的文献只应用了中医治疗,56.75%的文献应用了中西医结合疗法,只有1篇(0.14%)文献报告了除中医以外的补充替代疗法:多功能腰围的应用。结局指标分为6个方面:比率、量表、拉塞格征、生物学特性、血液指标和其他测量结果,其中有效率、优良率、视觉模拟量表(VAS)和日本骨科协会(JOA)评分排名前四。结论:中医疗法被广泛应用于腰椎间盘突出症的治疗。建议研究者按CONSORT声明来开展研究,以保证方法的质量。
Objective: To evaluate the available randomized controlled trials(RCTs) reports of complementary and alternative medicine(CAM) in treating lumbar disc herniation(LDH). Methods: Six Chinese and English databases were selected for research from establishment day to December 2013. Qualified RTCs reports were selected for further data extraction and verification conducted by two researchers. Results: 662 qualified RCTs reports from 1990 to 2012 that met the inclusion criteria were included, even though most of which were under low methodological quality. Among them, 43.11% reported the application of traditional Chinese medicine(TCM) therapy only, and 56.75% reported the application of integration of traditional and western medicine, only 1(0.14%) study reported the application of multi-function waistline. Outcome measures were divided into 6 aspects: rates, scales, Lasegue sign, biological characteristics, blood indicators and other outcomes, in which the top 4 reported were effective rate, excellent rate, visual analogue scale(VAS) and Japanese orthopaedic association(JOA) score. Conclusion: TCM has been extensively used in treating LDH, and meanwhile Chinese materia medica, massage and acupuncture in treating LDH has a bright feature. Researches following consolidated standards of reporting trials(CONSORT) can guarantee methodological quality effectively.
