柴芍承气汤对急性重症胰腺炎患者微循环的影响 被引量:13

Effect of Chaishao Chengqi decoction on microcirculation of severe acute pancreatitis patients

摘要 目的通过观察重症急性胰腺炎(SAP)治疗前后内皮素-1(ET-1)、一氧化氮(NO)、血小板α颗粒膜蛋白(GMP-140)、血栓烷B2(TXB2)和血管性假血友病因子相关抗原(v WF:Ag)浓度的变化,探讨柴芍承气汤对SAP患者微循环的影响。方法将100例符合标准的SAP患者随机分为A组(50例)和B组(50例)。另选取30例健康志愿者作为对照组。A组采用常规治疗方案,B组采用常规治疗+柴芍承气汤治疗方案。使用放射免疫法(RIA)测定ET-1;使用硝酸还原酶法测定血清NO含量;使用ELISA法测定血清GMP-140和TXB2。使用单克隆酶联免疫吸附法测定血清v WF:Ag浓度。结果在A组和B组SAP患者中,血清GMP-140、TXB2、v WF:Ag、ET-1浓度均高于对照组(P<0.01);血清NO浓度均低于对照组(P<0.05)。A组患者治疗后血清GMP-140、TXB2、NO、v WF:Ag及ET-1浓度与治疗前比较无明显差异(P>0.05);B组患者治疗后血清GMP-140、TXB2、v WF:Ag、ET-1浓度均显著低于治疗前(P<0.05),而血清NO显著高于治疗前(P<0.05)。结论血清NO、ET-1、GMP-140、TXB2、v WF:Ag参与了SAP微循环障碍的发生及发展过程;柴芍承气汤能够降低微血管痉挛,增加微循环灌注,抑制微血栓的形成,从而改善SAP患者的微循环障碍,阻止SAP恶化,改善预后,提高治愈率。 Objective To explore the effect of Chaishao Chengqi decoction on microcirculation of severe acute pancreatitis( SAP) patients through observing the endothelin-1( ET-1),( nitric oxide) NO,granule membrane protein-140( GMP-140),thromboxane B2( TXB2) and von Willebrand factor related antigen( v WF: Ag) before and after treatment. Methods One hundred SAP patients were randomly divided into group A( 50) and group B( 50),another 30 healthy volunteers were selected as a control group. Patients in A group were treated with conventional therapy,and B group with conventional therapy + Chaishao Chengqi decoction. Serum ET-1 was determined by radioimmunoassay( RIA). Serum NO content was measured by nitrate reductase method. Serum GMP-140 and TXB2 were tested by enzyme-linked immuno sorbent assay( ELISA) method. The concentration of serum v WF: Ag was tested by monoclonal enzyme-linked immunosorbent assay. Results The concentrations of GMP-140,TXB2,v WF: Ag and ET-1 of SAP patients in A group and B group were significantly higher than those in the control group( P〈0. 01). The serum NO concentration of SAP patients in A group and B group was significantly lower than that in the control group( P〈0. 05). After treatment,the concentrations of GMP-140,TXB2,NO,v WF: Ag,ET-1 of group A had no significant difference compared with those before treatment( P〉0. 05). The concentrations of GMP-140,TXB2,v WF: Ag,ET-1 of group B were significantly lower compared with those before treatment( P〈0. 05),and the concentration of serum NO of group B was significantly higher compared with that before treatment( P〈0. 05). Conclusions Serum NO,ET-1,GMP-140,TXB2 and v WF: Ag participate in the development process of SAP microcirculation disturbance. Chaishao Chengqi decoction can reduce microvascular spasm,increase the perfusion of microcirculation,and inhibit the formation of micro thrombosis,then improve the microcirculation disturbance of SAP patients,improve the prognosis and the cure rate of SAP.
