摘要
目的探讨丝裂原活化蛋白激酶激酶4(MAP2K4)、丝裂原活蛋白激酶1(MAPK1)基因多态性与缺血性中风风痰瘀阻证、气虚血瘀证的易感性及血脂水平的关联。方法采用病例对照研究方法,纳入缺血性中风风痰瘀阻证311例、气虚血瘀证284例、正常对照者605例。采用Sequenom技术进行基因分型,运用实时荧光定量法PCR(RT-PCR)检测外周血MAP2K4、MAPK1的mRNA表达水平和血脂水平。采用非条件Logistic回归模型分析基因型与缺血性中风易感性的关联,采用线性回归分析各位点多态性与血脂水平的关联。结果 MAPK1基因rs9340多态性与男性缺血性中风风痰瘀阻证易感性显著关联(P=0.047),而MAPK1基因rs6928多态性、MAP2K4基因rs3826392多态性与缺血性中风风痰瘀阻证、气虚血瘀证的易感性无关联(P>0.05)。携带rs6928 CC基因型的风痰瘀阻证患者的MAPK1 mRNA表达水平显著低于其他基因型携带者(P=0.006)。在缺血性中风气虚血瘀证中,MAPK1基因rs6928多态性与患者的甘油三酯(TG)、高密度脂蛋白(HDL)水平均有显著关联(P<0.05),rs9340与患者TG水平的关联具有意义(P<0.05),但未观察到MAP2K4基因rs3826392多态性与缺血性中风风痰瘀阻证、气虚血瘀证的血脂水平具有关联(P>0.05)。结论 MAPK1基因rs9340多态性可能影响中国男性缺血性中风风痰瘀阻证的发生,MAPK1基因多态性影响缺血性中风气虚血瘀证的血脂代谢。
Objective To investigate the association between the gene polymorphism of mitogen activated protein kinase 4( MAP2K4) and mitogen activity protein kinase 1( MAPK1) and the susceptibility of ischemic stroke with wind phlegm and blood stasis syndrome,Qi deficiency and blood stasis syndrome and the blood lipid level. Methods A case control study method was applied,and there were 311 cases of ischemic stroke with phlegm and blood stasis syndrome,284 cases with Qi deficiency and blood stasis syndrome,and 605 cases of normal control subjects included.Gene typing was performed by using Sequenom technology,the expression level of MAP2K4 and mRNA of MAPK1 in peripheral blood and the blood lipid level were detected by real-time fluorescence quantitative PCR( RT-PCR).Results The MAPK1 gene rs9340 polymorphism was significantly associated with the susceptibility to wind phlegm and blood stasis syndrome in male patients with ischemic stroke( P = 0. 047),however there was no association between MAPK1 gene rs6928 polymorphism and MAP2K4 gene rs3826392 polymorphism and the susceptibility of ischemic stroke with wind phlegm stasis syndrome,and Qi deficiency and blood stasis syndrome( P 〉 0. 05). The expression level of mRNA MAPK1 was significantly lower in patients with wind phlegm and blood stasis syndrome carrying rs6928 CC genotype than other genotype carriers( P = 0. 006). In ischemic stroke patients with Qi deficiency and blood stasis syndrome,MAPK1 gene rs6928 polymorphism was significantly associated with levels of triglyceride( TG) and high density lipoprotein( HDL)( P 〈 0. 05),and there was significant association between rs9340 and TG levels( P 〈 0. 05),but the association between MAP2K4 gene rs3826392 polymorphism and the blood lipid levels in ischemic stroke patients with phlegm and blood stasis syndrome,Qi deficiency and blood stasis syndrome were not found( P 〉 0. 05). Conclusion MAPK1 gene rs9340 polymorphism may affect the occurrence of ischemic stroke with wind phlegm and blood stasis syndrome in Chinese male. MAPK1 gene polymorphism affects the blood lipid metabolism in ischemic stroke with Qi deficiency and blood stasis.
