FNA在^(99)Tc^mO_4^-甲状腺显像单发“冷”结节良恶性鉴别诊断中的临床价值 被引量:2

摘要 目的探讨FNA在99TcmO-4甲状腺显像单发"冷"结节良恶性鉴别诊断中的临床价值。方法回顾性分析116例99TcmO-4甲状腺显像单发"冷"结节患者FNA和手术病理结果,以手术病理为金标准,计算FNA的灵敏度、特异度、阳性预测值、阴性预测值和诊断准确性。结果 1116枚单发"冷"结节中,手术病理结果为良性86例(74%),恶性30例(26%)。恶性和良性结节患者在年龄、性别及血清TSH水平方面均无统计学差异(P=0.238,0.713,0.426)。2FNA的灵敏度为66.7%,特异度为97.7%,阳性预测值为90.9%,阴性预测值为89.4%,准确度为90.3%,与手术病理结果一致性Kappa值为0.732。结论 FNA在99TcmO-4甲状腺显像单发"冷"结节良恶性鉴别诊断中具有很高诊断效能和准确性,细胞病理学和组织病理学也有很好的一致性;FNA是一种很好的甲状腺单发"冷"结节良恶性诊断的首选检查方法。 Objective Preliminary discussion of diagnostic value of fine needle aspiration(FNA) in differentiating benign from malignant in single cold nodule detected by99TcmO-4thyroid scintigraphy.Methods The cytopathology after FNA and histopathology after operation from 116 patients with single cold nodule were analyzed retrospectively.The sensitivity,specificity,positive predictive value(PPV),negative predictive value(PPV) and accuracy of FNA were calculated by taking the histopathology as a golden standard.Kappa statistic was used to measure agreement between cytology and histopathology.Results Out of 116 patients,eighty-six benign nodules and thirty malignant nodules were detected.There was no statistically significant difference between benign and malignant nodules in age,sex and the level of serum TSH.The sensitivity,specificity,PPV,NPV and accuracy of FNA was 66.7%,97.7%,90.9%,89.4%,90.3%,respectively.The kappa statistic value was 0.732.Conclusion FNA of single cold thyroid nodules had a high diagnostic value and it was reliable.It was highly agreeable between cytopathology and histopathology.Therefore,FNA is a preferred method in differentiating benign from malignant for single cold thyroid nodules.
