摘要
目的:探讨原发性肝癌患者超声表现与中医分型的关系分析。方法:选择2011年4月—2014年3月医院收治的原发性肝癌患者104例作为研究对象,按照中医分型将其分为气滞血瘀型、脾虚湿困型、湿热聚毒型、肝肾阴虚型,比较不同中医分型之间超声表现情况。结果:气滞血瘀型患者超声表现分型主要为块状型(65.63%)、脾虚湿困型患者主要表现为小癌型(78.57%)、湿热聚毒型患者主要表现为弥漫型(52.00%)、肝肾阴虚型患者主要表现为结节型(63.16%),各中医分型患者超声表现构成比比较具有统计学差异(P<0.05);不同中医分型患者之间肝动脉血流动力学参数(Vmax、PI、RI)比较均具有统计学显著性差异(P<0.05);不同中医分型患者门静脉血流动力学参数D、Q上比较差异具有统计学意义(P<0.05),而TAMX比较则不具有统计学意义(P>0.05)。结论:对于原发性肝癌患者而言,不同中医辨证分型之间具有着不同的超声表现,且在肝脏血流动力学上具有着较为明显的区别,临床上可将超声诊断技术与中医辩证相结合,从而达到优势互补的作用。
Objective: To investigate the relationship between hepatocellular carcinoma on ultrasonography and TCM syndrome types. Methods: During April 2011 to March 2014 in our hospital,a total of 104 cases of primary liver cancer patients,as the research object,according to the TCM syndrome types were divided into qi stagnation and blood stasis,spleen deficiency with dampness,damp-heat accumulation and yin deficiency of liver and kidney,comparing different TCM types ultrasonic manifestations. Results: The ultrasonic manifestations of qi stagnation and blood stasis type is mainly blocking type( 65. 63%),spleen dampness type mainly manifested as small cancer type( 78. 57%),damp-heat accumulation mainly manifested as nodular type( 52%),yin deficiency of liver and kidney mainly manifested as nodular type( 63. 16%). The ratio of different ultrasonographic findings was significant( P〈0. 05). Different TCM syndrome types patients' hepatic artery hemodynamic parameters( Vmax,PI,RI) were statistically significant( P〈0. 05). And those patients' portal vein hemodynamics parameters D and Q had significant difference( P〈0. 05) and TAMX was not significant( P〉0. 05). Conclusion: For the patients with primary liver cancer,different TCM syndrome types have different ultrasonographic findings and hepatic hemodynamics with apparent differences. The ultrasound technology and traditional Chinese medicine can be combined in clinic so as to achieve the complementary function.
