急性失代偿性心力衰竭患者接受重组人脑利钠肽联合硝普钠治疗后的血流动力学及心功能评价 被引量:33

摘要 目的:评价急性失代偿性心力衰竭患者接受重组人脑利钠肽联合硝普钠治疗后的血流动力学及心功能水平。方法:118例急性失代偿性心力衰竭患者随机分为观察组及对照组各59例,对照组患者接受临床心力衰竭常规治疗,观察组患者接受重组人脑利钠肽联合硝普钠治疗,对比治疗12h后两组患者的血流动力学、心功能及循环因子水平差异。结果:观察组患者经治疗12h后,中心静脉压、右心房压、肺毛细血管楔压值,循环血IL-6、hsCRP、ST2、NT-proBNP、cTnⅠ水平均低于对照组患者,左心GLS、GCS、GSRs、GSRe、GSRa、ROT、ROTR水平高于对照组患者(均P<0.05)。结论:急性失代偿性心力衰竭患者接受重组人脑利钠肽联合硝普钠治疗,在优化的血流动力学及心功能等方面均有较大优势。 Objective:To evaluate the effect of recombinant human brain natriuretic peptide combined with sodium ni-troprusside therapy on hemodynamics and cardiac function levels in patients with acute decompensated heart failure.Methods:A total of 1 18 patients with acute decompensated heart failure were randomly divided into observation group and the control group (n =59).Control group received clinical conventional therapy for heart failure,observation group received recombinant human brain natriuretic peptide combined with sodium nitroprusside therapy,and the differences in hemodynamics,cardiac function and circulation factor levels were compared between two groups after 12 hours of treatment.Results:After 12 hours of treatment,central venous pressure,right atrial pressure and pulmonary capillary wedge pressure values as well as circulating blood IL-6,hsCRP,ST2,NT-proBNP and cTnⅠ levels of observation group were lower than those of control group,and left heart GLS,GCS,GSRs,GSRe,GSRa,ROT and ROTR levels were higher than those of control group (P 〈0.05).Conclu-sions:Recombinant human brain natriuretic peptide combined with sodium nitroprusside therapy for patients with acute decom-pensated heart failure has significant advantages in optimizing hemodynamics,cardiac function and other aspects.
