摘要
目的探讨真菌性上颌窦炎功能性鼻内镜术后黏膜转归的时间规律。方法收集80例上颌窦病变患者临床资料,在实施功能性鼻内镜鼻窦术后的第2、3、4、6、8、12、16、24周采用Lund-Kennedy内镜评分方法对上颌窦黏膜形态进行评估,比较不同时期真菌性上颌炎与慢性上颌窦炎窦腔黏膜形态的评分数值和完成上皮化比例。结果术后第4、8周真菌性上颌窦炎病变黏膜形态评分分别为（2.21±0.98）、（2.75±0.78）分,慢性上颌窦炎病变黏膜形态评分分别为（1.63±0.97）、（2.06±0.44）分,差异均有统计学意义（χ-2=36.74,P=0.043〈0.05;χ-2=40.46,P=0.046〈0.05）;术后第3、4、24周上颌窦黏膜的转归过程呈明显分离趋势,完成上皮化比例差异有统计学意义（Z=-3.153,P=0.002〈0.05;Z=-2.132,P=0.033〈0.05;Z=-3.922,P=0.000〈0.05）。结论真菌性上颌窦炎与慢性上颌窦炎功能性鼻内镜鼻窦术后黏膜转归存在时间差异,真菌性上颌窦炎术后黏膜评分及上皮化比例高于慢性上颌窦炎,且上皮化时间较短。
Objective To explore the time regularity of mucous transitionary course of fungal maxillary sinusitis after functional endoscopic sinus surgery. Methods Eighty maxillary sinusitis patients following functional endoscopic sinus surgery were collected. The endoscopic appearances of maxillary mucous were respectively evaluated with Lund- Kennedy scoring system in 2,3,4,6,8,12,16,24 weeks. Then the endoscopic scores and epithelization proportions of fungal maxillary sinusitis and chronic maxillary sinusitis in different stages were analyzed and compared. Results In the 4,8 weeks after operation,the mucosa scores of fungal maxillary sinusitis were（ 2. 21 ± 0. 98）,（ 2. 75 ± 0. 78） scores,chronic maxillary sinusitis were（ 1. 63 ±0. 97）,（2. 06 ± 0. 44） scores,the differences were statistically significant（ χ-2= 36. 74,P = 0. 043 0. 05; χ-2= 40. 46,P =0. 046 0. 05）. In the 3,4,24 weeks after operation,the transitionary course of maxillary mucous showed a separation tendency,and epithelization proportions the differences in epithelization proportions were statistically significant（ Z =- 3. 153,P =0. 002 0. 05; Z =- 2. 132,P = 0. 033 0. 05; Z =- 3. 922,P = 0. 000 0. 05）. Conclusion The transitionary course of fungal maxillary sinusitis and chronic maxillary sinusitis after functional endoscopic sinus surgery shows significant timing differences. The mucosa score and epithelization proportions of fungal maxillary sinusitis are higher and theepithelization time was shorter than chronic maxillary sinusitis.
