孕期体重增加对妊娠晚期孕妇血脂影响及其与新生儿发育相关性研究 被引量:35

Effect of gestational weight gain on the correlation between maternal lipid level and newborn body size in the women in late pregnancy

摘要 目的 探讨孕期体重增加对孕妇孕晚期血脂水平的影响,及其与新生儿体重指数之间的相关性。方法 收集2012年1月至2013年3月在中国医科大学附属盛京医院分娩、孕前体重指数正常孕妇共计566例。按孕妇孕期体重增加分成3组:〈11.5 kg组,11.5-16.0 kg组,〉16.0 kg组。产前1周检测各组产妇血清三酰甘油、胆固醇、高密度脂蛋白、低密度脂蛋白水平。收集新生儿资料信息。结果 〈11.5 kg组新生儿出生体重、身长、头围与孕妇妊娠晚期血清三酰甘油水平呈正相关(r=0.286,P〈0.01;r=0.214,P=0.011;r=0.241,P〈0.01),与高密度脂蛋白水平呈负相关(r=-0.388,P〈0.01;r=-0.229,P〈0.01;r=-0.273,P〈0.01)。〉16 kg组新生儿出生体重与孕妇妊娠晚期血清三酰甘油水平呈正相关(r=0.210,P=0.01)。11.5-16.0 kg组新生儿身长及出生体重与母体孕晚期血脂水平未见明显相关性。结论 母体妊娠期体重增加过多,孕晚期三酰甘油水平升高,可能与巨大儿发生相关。 Objective To detect the relationship between the lipid profile in late pregnancy and the newborn birth weight in different gestational weight gain groups. Methods Clinical data of 566 women with normal BMI who deliv- ered in Shengjing Hospital between Jan. 2012 and Mar. 2013 were collected and divided into 3 groups: 〈11.5 kg group, 11.5-16.0 kg group, 〉16.0 kg group. The TG, TC, HDL and LDL were detected within one week before delivery. The da- ta of newborn were collected after delivery.Results The birth weight, body length and head circumference of newborn in 〈 11.5 kg group were positively related to maternal TG(r=0.286, P〈0.0 1 ; r=0.214, P=0.011; r=0.241, P〈0.0 1) and nega- tively related to maternal HDL(r= - 0.388, P〈0.01 ;r= - 0.229, P〈0.01; r= - 0.273, P〈0.01). The birth weight of newborn in 〉16 kg group were positively related to maternal TG (r=0.210, P=0.01). There was no significant difference between body length, birth weight and maternal lipid level in 11.5-16.0 kg group.Conclusion Excessive gestational weight gain may induce high triglyceride in the third trimester, which may be related to macrosomia.
