摘要
目的探讨异氟烷麻醉对颅脑外伤手术患者血糖、血脂和肾功能的影响。方法选取2014年7月至2015年7月收治的颅脑外伤手术患者90例,按照随机数字表法将患者分为研究组和对照组,每组45例。对照组给予丙泊酚和芬太尼麻醉,研究组给予异氟烷麻醉,比较两组术前、术中1 h、术中3 h以及术后2 h血糖、三酰甘油水平,并比较两组术前、术后1 d、术后3 d和术后7 d的尿量、血肌酐、尿素氮以及尿酸水平。结果研究组术中3 h血糖水平达到高峰然后下降,对照组术后2 h达到高峰,差异具有统计学意义(P<0.05);研究组三酰甘油水平较平稳,对照组术中1,3 h明显升高,差异具有统计学意义(P<0.05)。两组术后1天尿量开始增加,术后第7天尿量有所降低,两组比较差异无统计学意义(P>0.05);两组术后1天血肌酐和尿素氮升高,与术前比较差异具有统计学意义(P<0.05),两组比较差异具有统计学意义(P<0.05);术后7天血肌酐和尿素氮水平降低,与术前比较差异无统计学意义(P>0.05),两组比较差异无统计学意义(P>0.05)。两组尿酸水平无明显变化,差异无统计学意义(P>0.05)。结论异氟烷麻醉应用于颅脑外伤手术对血糖具有一定影响,对三酰甘油无明显影响,对肾脏有保护作用。
Objective To study the effect of isoflurane anesthesia on blood glucose, blood lipid and renal function in patients with cran- iocerebral traum. Methods 90 cases with craniocerebral trauma operation were selected from July 2014 to July 2015 and divided into study group and control group according to the random number table method. The control group was given propofol and fentanyl anes- thesia, the study group received isoflurane anesthesia. The blood glucose and triglyceride preoperative, intraoperative 1, 3, and 2 h of the two groups were compared, and urine volume, serum creatinine, urea surgery of two groups were compared. Results The blood glucose nitrogen and uric acid preoperative, postoperative 1, 3 and 7 d after of intraoperative 3 h reached the peak and then decreased in the study group, the blood glucose of postoperative 2 h reached the peak in the control group, the difference was statistically significant( P 〈 0.05); triglyceride was more stable in the study group, intraoperative triglyceride 1 h and 3 h in the control group were significantly elevated, the difference was statistically significant(P 〈 0. 05); the urine volume of the first day in the two groups began to increase after operation, and the urine volume was decreased after the seventh day, there was no statistical significance between the two groups (P 〉 0.05); blood creatinine and urea nitrogen increased on the first day after operation in the two groups, the difference was statisti- cally significant compared preoperative( P 〈 0.05), the difference between the two groups was statistically significant(P 〈 0. 05); blood creatinine and urea nitrogen were decreased after the seventh day of operation, there was no significant difference compared preoperative (P 〉 0. 05), and there was no significant difference between the two groups( P 〉 0. 05); there was no obvious change in uric acid of the two groups, the difference was statistically significant(P 〉 0. 05). Conclusion There is a certain effect of isoflurane anesthesia on blood glucose, there is no significant effect on the triglyceride, and there is protective effect of isoflurane on the kidney.
