摘要
目的探讨甲状腺手术术后血钙的变化趋势,为临床实践提供指导。方法选取2013年6月-2015年12月本科收住的470例甲状腺手术患者作为研究对象。分别于术前1天、术后1天、术后3天和术后7天对患者的血清钙进行检测,并进行统计学分析。结果术后1天,血钙降低者70例,占17.95%;术后3天,血清钙降低者98例,占25.13%;术后7天,血清钙降低者12例,占3.08%。低血钙发生率总体上呈现出先升高,后降低的趋势。术后1天和术后3天患者的血钙较术前均显著下降,与术前比较差异均具有统计学意义(P<0.05)。术后3天血钙含量最低,之后呈现上升趋势。术后7天血钙逐渐恢复到正常水平,与术前相比差异无统计学意义(P>0.05)。术后1天和3天不同手术组患者的血钙较术前均出现不同程度的降低,与术前比较差异均具有统计学意义(P<0.05)。而双侧全切组的血钙水平明显低于其他各组,差异也均具有统计学意义(P<0.05)。术前低血钙组和血钙正常组的甲状旁腺激素(PTH)水平相当,差异无统计学意义(P>0.05);术后1天,两组的甲状旁腺激素(PTH)均明显降低,两组比较显示,低血钙组患者的甲状旁腺激素(PTH)水平明显低于血钙正常组,差异具有统计学意义(P<0.05)。结论甲状腺手术后血钙的变化具有重要临床意义。
Objective To investigate the change trends of serum calcium after thyroid surgery, aimed at providing guidance for clinical practice. Methods 470 cases of thyroid surgery patients treated in our hospital from June 2013 to December 2015 were enrolled as subjects. Serum calcium was detected and analyzed before surgery, one day, 3 days and 7 days after the operation respectively. Results Serum calcium decreased in 70 cases at the first day after surgery, accounting for 17.95%. Serum calcium decreased in 98 decreased at the third postoperative day, accounting for 25.13%. Serum calcium reduced in 12 cases at 7 days after surgery, accounting for 3.08%. After 1 day and three days, serum calcium in patients significantly decreased when compared with preoperative level(P〉0.05). Serum phosphorus was minimum after 3 days, then showed a rising trend. Serum calcium gradually returned to normal levels after seven days. However, difference was not statistically significant(P〈0.05). Serum calcium reduced at 1and 3 days after surgery in all groups compared with preoperative and the difference were statistically significant(P〈0.05). The serum calcium level in bilateral total resection group was significantly lower than the other groups and the differences were statistically significant(P〈0.05).PTH levels in hypocalcemia group and the normal group were similar before surgery, the difference was not statistically significant(P〈0.05). At the first postoperative day, PTH in two groups were significantly decreased. PTH levels in hypocalcemia group were significantly lower than normal calcium group, the difference was statistically significant(P〈0.05).Conclusion Change trends of serum calcium after thyroid surgery had clinical significance.
