2015年巴中市巴州区碘缺乏病现况调查 被引量:3

Cross-sectional Investigation on Iodine Deficiency Disorders in Bazhou District of Bazhong City

摘要 目的掌握巴中市巴州区碘缺乏病病情、人群碘营养和防治措施落实现况,为综合评价防治成效和实施针对性的防治措施提供依据。方法 2015年,采用单纯随机抽样:在巴州区东、西、南、北和中5个方位各抽取1个乡(镇)作为调查点,在每个调查点中心小学抽取80名8-10岁儿童(年龄均衡、男女各半)开展甲状腺肿大情况调查,采集其中10人1次随机尿样检测尿碘同时采集其家中食用盐检测盐碘并调查其家庭日人均食盐摄入量;在每个调查点抽取40岁以下育龄妇女、孕妇、哺乳期妇女及其0-2岁婴幼儿各10人采集1次随机尿样检测尿碘同时采集其家中食用盐检测盐碘;在每个调查点抽取不相邻2个村,每村采集1份日常生活饮用水检测水碘并调查1个村级碘盐销售点情况;在供应巴州区碘盐的盐业公司仓库采集9份盐样(上下半年各1次)评价碘盐产品批次合格情况。结果甲状腺肿大率0.25%;尿碘中位数228.1μg/L,〈50μg/L比例为0.8%;盐碘中位数29.1 mg/kg,碘盐覆盖率100%,合格碘盐食用率97%;水碘中位数6.3μg/L,范围(2.8-9.2)μg/L;居民家庭日人均食盐摄入量8.5 g;碘盐产品含碘量均值(24.96±2.16)mg/kg;碘盐销售点所售食用盐均为碘盐。结论巴州区自然环境普遍缺碘,防治措施得到有效落实,人群碘营养水平总体适宜并持续保持碘缺乏病消除状态。 Objective To provide evidences for comprehensive evaluation of control effects and implementation of more targeted prevention measures by mastering the epidemic status of iodine deficiency disorders( IDD),the level of iodine nutrition among the population and the fulfillment status of prevention measures Bazhou District.Methods One town was selected in each of five directions( east,west,south,north and central) in Bazhou District by simple random sampling. In each town,80 children aged 8-10 were randomly selected from central primary school for thyroid examination; additionally,in 10 of them,urinary iodine assay was performed,salt samples were collected from their homes and salt iodine levels were determined,and daily salt intake per capita was investigated. Ten women of child- bearing potential below 40 years old,pregnant women,lactating women and their 0-2 years old infants or young children were selected in each town for urinary iodine and salt iodine assay. Two nonadjacent villages were randomly selected in each town,one drinking water sample was collected from each village for the iodine content determination,and sales of iodized salt was also investigated in each village.Nine iodized salt samples were collected in five directions from warehouses of salt companies for conformity assessment. Results The incidence rate of thyroid enlargement in children aged 8-10 was 0. 25%; the median urinary iodine level was 228. 1μg / L and the proportion of people with urinary iodine 50μg / L was 0. 8%; the iodized salt coverage rate,the consumption rate of qualified iodized salt and the median of iodized salt were respectively 100%,97% and 29. 1mg / kg; the iodine in water was in range of 2. 8μg / L to 9. 2μg / L,and the median of them was 6. 3μg / L; the daily salt intake per capita of the households surveyed was 8. 5g; the mean of iodized salt in the warehouse surveyed was( 24. 96 ±2. 16) mg / kg; no non-iodized salt was found. Conclusion The natural environment is generally deficient of iodine in Bazhou District,but control and prevention measures for IDD have been implemented effectively,iodine nutrition among population keeps at a suitable level and elimination of IDD is maintained.
