摘要
目的探讨应用连续无创血压监测系统(continuous non-invasive arterial pressure,CNAP)所测的收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)、每搏量变异度(SVV)、脉压变异度(PPV)、心排出量(CO)和每搏量(SV)等血流动力学参数与Flotrac/Vigileo获得的相关参数的一致性。方法 25例择期行全麻下开腹手术的患者,年龄18~60岁,美国麻醉医师协会(ASA)麻醉风险分级Ⅰ~Ⅱ级者,静脉全麻诱导后于患者左手进行桡动脉穿刺,连接Flotrac/Vigileo,左手食指与中指连接Li DCOrapid(CNAP)双指套传感器,在手术过程中每隔5 min记录SBP、DBP、MAP、SVV、PPV、CO、SV等血流动力学参数。结果 CNAP组SBP低于Flotrac/Vigileo组(P〈0.01),CNAP组DBP和PPV高于Flotrac/Vigileo组(P〈0.05);2组的MAP、SVV、CO、SV的差异无统计学意义。2组的SBP、DBP相关系数r=0.763,r=0.700,P均〈0.01,而PPV的相关系数r=0.662,P〈0.01。结论 CNAP与Flotrac/Vigileo具有相似的临床应用价值。但也应注意SBP、DBP和PPV存在的差异。
Objective To evaluate the association between systolic blood pressure(SBP),diastolic blood pressure(DBP),mean arterial pressure(MAP),stroke volume variation(SVV),pulse pressure variation(PPV),cardiac output(CO)and stroke volume(SV)monitored by non-invasive and invasive monitoring.Methods Twenty-five ASA classⅠ-Ⅱpatients of both genders,aged 18-60 years old,who underwent laparotomy were received general anesthesia.After intravenous anesthesia induction,the radial artery puncture was used in each patient's left hand,connected the Flotrac/Vigileo,left index finger and middle finger of left hand was connected Li DCOrapid(CNAP)double refers to the set of sensors.SBP,DBP,MAP,SVV,PPV,CO,SV,and hemodynamic parameters in the process of operation were recorded every5 minutes.Results The SBP was lower in the CNAP group than in the Flotrac/Vigileo group(P〈0.01).The DBP and PPV(P〈0.01)were higher in the CNAP group than in the Flotrac/Vigileo group(P〈0.05).There were no differences in the MAP,SVV,CO and SV between two groups.There were good correlations between the two groups of SBP(r = 0.763,P〈0.01),DBP(r = 0.700,P〈0.01),PPV(r = 0.662,P〈0.01).Conclusion Li DCOrapid(CNAP)may provide similar potential benefits like Flotrac/Vigileo.But it should be paid attention to the differences of SBP,DBP and PPV.
