摘要
目的：探讨甲氨喋呤三联疗法对类风湿关节炎患者血清sB7-H3、INF-r及IL-2水平的影响。方法：选取某院2014年9月~2015年2月收治的50例类风湿关节炎患者为研究对象,采取甲氨喋呤三联疗法;49例类风湿关节炎患者为对照组,采取甲氨喋呤二联疗法;36例正常人为健康组,采用ELISA（enzymelinkedimmunosorbentassay,酶联免疫吸附剂测定）夹心法检测患者及正常人群血清中sB7-H3、INF-r及IL-2的表达水平。结果：甲氨喋呤三联疗法的总有效率为96%,具有统计学差异（χ2=8.564,P=0.004）。研究活动组患者血清中sB7-H3、INF-r及IL-2水平较正常人群有统计学差异（P〈0.05）;研究缓解组患者血清中sB7-H3、INF-r及IL-2水平较正常人群无统计学差异（P〉0.05）;研究缓解组较活动组患者血清中sB7-H3、INF-r及IL-2水平均有显著降低,均具有统计学意义（P〈0.05）。研究组的关节疼痛指数、关节肿胀指数及关节压痛指数较对照组均显著下降（P〈0.001）,且各指标在两组间均有统计学差异（P〈0.001）。研究组的皮疹、局部疼痛、胃肠道反应、谷丙转氨酶水平增加的发生均显著少于对照组,差异均具有统计学意义（P〈0.05）。结论：甲氨喋呤三联疗法能够降低患者血清中sB7-H3、INF-r及IL-2水平,降低症状体征发生率,有助于患者病情恢复,降低不良反应发生率,值得临床推广应用。
Objective:To investigate the effect of combination therapy of methotrexate(MTX)on the level of sB7-H3,INF-r and IL-2of patients with rheumatoid arthritis.Methods:50cases of patients with rheumatoid arthritis in our hospital from September 2014 to February 2015 were taken as research object and they were treated with Methotrexate triple therapy.Other 49 cases of patients with rheumatoid arthritis were treated with Methotrexate bigeminy therapy;the control group consisted of 36 healthy cases,who were treated with ELISA.Serum sB7-H3,INF-r and IL-2in patients and healthy cases were performed using ELASA.Results:After treating with MTX,the total effectiveness was 96%,and it was significantly different(χ2=8.564,P=0.004).The levels of serum sB7-H3,INF-r and IL-2among active and healthy group were significantly different(P<0.05);The levels of serum sB7-H3,INF-r and IL-2among non-active and healthy group were not significantly different(P>0.05).Besides,the levels of serum sB7-H3,INF-r and IL-2in non-active group decreased significantly than active group and they were significantly different(P<0.05).Triple therapy of rheumatoid arthritis can significantly reduce the number of joint pain,the number of swollen joist in patients and number of joint tenderness(P<0.001).The treatment group were rash,local pain,gastrointestinal reaction,alanine aminotransferase levels increased significantly less than the control group,the differences were statistically significant(P<0.05).Conclusion:The combination therapy of methotrexate can decrease the levels of serum sB7-H3,INF-r and IL-2in RA patients,reduce the incidence of adverse reactions,help to restore patients' health and it is worthy applying in the future.
