摘要
目的探讨难治性癫痫患者抑郁状态与症状自评量表(Symptom checklist-90,SCL-90)评分、血清学指标之间的相关性。方法将80例难治性癫痫患者,根据汉密尔顿抑郁量表(HAMD)评分分为抑郁障碍组和非抑郁障碍组。检测2组患者血清中脑源性神经营养因子(BDNF)、神经生长因子(NGF)、γ-氨基丁酸(GABA)、谷胱甘肽巯基转移酶(GST)、P糖蛋白、多药耐药相关蛋白(MRP)水平,并对患者进行SCL-90评分。结果抑郁障碍组患者血清NGF水平明显低于非抑郁障碍组(P<0.05),血清GABA水平明显高于非抑郁障碍组(P<0.05)。抑郁障碍组患者SCL-90评分明显高于非抑郁障碍组,表现在躯体化、强迫症状、人际关系敏感、抑郁、焦虑、敌对、恐怖、偏执、精神病性等因子维度(P<0.01)。抑郁障碍组患者HAMD评分与血清NGF水平呈负相关(r=-0.256,P<0.05),与GABA水平呈正相关(r=0.259,P<0.05),与SCL-90评分呈显著正相关(r=0.720,P<0.01)。结论难治性癫痫患者抑郁障碍程度与细胞因子水平之间具有一定的相关性。难治性癫痫患者除抑郁障碍之外,可能还存在其他的心理障碍。
Objective To investigate the correlation between depression status and symptom checklist-90 (SCL-90) scores and serum markers in patients with intractable epilepsy. Methods 80 inpatients with intractable epilepsy were divided into the depression group and non-depression group by Hamilton Depression Scale (HAMD) scores. Brain derived neurotrophic factor ( BDNF), nervegrowth factor( NGF ), γ-aminobutyric acid ( GABA ), Glutathione S-transferase ( GST), P-Glycoprotein levels and Multidrug resistance protein(MRP) were detected SCL-90 scores made in two groups. Results The levels of serum NGF in the depression group were significantly lower than that in the non-depression group( P 〈0.05 ). Serum GABA level was significantly higher than that of the non- depression group( P 〈 0. 05 ). The scores of SCL-90 of the depression group were significantly higher than that of the non-depression group, which were manifested in the factors, including physical status, compulsive symptoms, interpersonal sensitivity, depression, anxiety, hostility, terror, paranoia and psychosis(P 〈 0. 01 ). HAMD scores of the depression group were negatively correlated with serum NGF( r = - 0. 256, P 〈 0. 05 ) and positively with GABA level( r = 0. 259, P 〈 0. 05 ), also positively correlated with SCL-90 scores(r =0. 720, P 〈0.01 ). Conelusiort There is certain correlation between depression and cytokines in patients with intractable epilepsy. Other mental disorders may also exist besides depression in patients with intractable epilepsy.
