摘要 目的探讨分析血清同型半胱氨酸(Hcy)与自然流产之间的关系及B族维生素治疗高半胱氨酸血症的效果。方法采用病例对照研究方法,随机选取2014年1月至2016年1月于西北妇女儿童医院就诊的有自然流产病史的孕前检查病例200例,其中有复发性自然流产史(复发流产组)和单次自然流产史(单次流产组)的孕前检查病例各100例,对照收集同期在同一医院进行正常孕前检查者100例(对照组),测定其血清Hcy。对Hcy≥10μmol/L的138例自然流产史病例进行B族维生素治疗30天,比较治疗前后血清Hcy水平,并评估治疗结果。结果单次流产组、复发流产组及正常孕前检查对照组Hcy的检测值分别为:10.54±2.14μmol/L、12.76±2.38μmol/L和9.41±1.21μmol/L,单次流产组和复发流产组的Hcy均显著高于正常孕前检查对照组(F=10.67,P<0.05)。在Hcy≥10μmol/L的有效观察130例中,治疗前Hcy平均水平为11.38±2.12μmol/L,经B族维生素治疗后,Hcy平均水平为9.56±1.76μmol/L,较治疗前明显降低(t=9.32,P<0.05)。结论血清Hcy可以做为自然流产风险因素来检测,并可通过B族维生素有效降低血清高半胱氨酸水平来预防复发性自然流产。 Objective To investigate the relationship between homocysteine( Hcy) and natural abortion and the curative effect of vitamin B on Hcy. Methods With case-control study method,totally 200 cases with natural abortion history were selected in Northwest Maternal and Child Care Hospital,and they took prenatal examination during the period of January 2014 to January 2016,including 100 cases of recurrent natural abortion history( recurrent abortion group) and 100 cases of natural abortion history for one time( single abortion group).At the same period 100 cases for health pregnant examination in the hospital were selected( control group). The serum Hcy levels were detected. For 138 cases of natural abortion history with Hcy≥10umol / L,vitamin B treatment was provided for 30 days. Serum Hcy levels were compared before and after treatment,and therapeutic effect was evaluated. Results The Hcy detected level was 10. 54 ± 2. 14μmol / L,12. 76 ± 2. 38μmol / L and 9. 41 ± 1. 21μmol / L in single abortion group, recurrent abortion group and normal pregnant women,respectively,and those in single abortion group and recurrent abortion group were significantly higher than that in normal pregnant women( F = 10. 67,P〈0. 05). Among effectively observed 130 cases with Hcy≥10μmol/L,the average Hcy level before treatment was 11.38 ±2. 12μmol /L,and it decreased remarkably after vitamin B treatment( 9. 56 ± 1. 76μmol /L)( t = 9. 32,P〈0. 05). Conclusion Serum Hcy level can be detected as a risk factor of natural abortion,and vitamin B can reduce the level of Hcy effectively to prevent recurrent natural abortion.
